Hearing Aid Plan. Effective March 24, 2014, and for the duration of this agreement, coverage will be eight hundred ($800.00) dollars, paid once every thirty-six (36) months (employees and dependents) toward the cost of purchasing a hearing aid.

The Plan will provide for the purchase of Hearing Aids obtained on the written prescription of a physician certified as an audiologist, not exceeding five hundred ($500.00) dollars per person during the entire time the person is insured. There shall be no deductible payable by the employee.
