Health Coaching. As more fully described in the fee schedule below, to the extent Client receives Health Coaching, the applicable terms of Encompass 360’s Coaching Client Agreement are incorporated herein by reference as if fully set forth.

Health Coaching. Contractor shall provide access to a qualified health coach through email, phone, one-on-one, or group consultations. Health coach shall provide information and advice to Participants with respect to health care issues. The health coach will track and monitor utilization of this program and will submit a monthly utilization report to the City.
Health Coaching. ~ You can also earn points for participating in Ongoing Coaching following the close of biometric screening. Each quarter, the City will have a health coach from Froedtert onsite; you can meet with the coach to follow-up on the goals you set during biometric screening, or address whatever issues or impediments you may be experiencing as you work towards a healthier lifestyle.   • Health Care Operations Health Care Operations shall have the meaning set out in its definition at 45 C.F.R. § 164.501, as such provision is currently drafted and as it is subsequently updated, amended or revised.

  • Health Overcoming or managing one’s disease(s) as well as living in a physically and emotionally healthy way;

  • Extended Health Care Plan (a) The Employer shall pay the monthly premium for regular employees entitled to coverage under a mutually acceptable Extended Health Care Plan.

  • Orthodontics We Cover orthodontics used to help restore oral structures to health and function and to treat serious medical conditions such as: cleft palate and cleft lip; maxillary/mandibular micrognathia (underdeveloped upper or lower jaw); extreme mandibular prognathism; severe asymmetry (craniofacial anomalies); ankylosis of the temporomandibular joint; and other significant skeletal dysplasias. Procedures include but are not limited to: • Rapid Palatal Expansion (RPE); • Placement of component parts (e.g. brackets, bands); • Interceptive orthodontic treatment; • Comprehensive orthodontic treatment (during which orthodontic appliances are placed for active treatment and periodically adjusted); • Removable appliance therapy; and • Orthodontic retention (removal of appliances, construction and placement of retainers).

  • HEALTH CARE PLANS ‌ Notwithstanding the references to the Pacific Blue Cross Plans in this article, the parties agree that Employers, who are not currently providing benefits under the Pacific Blue Cross Plans may continue to provide the benefits through another carrier providing that the overall level of benefits is comparable to the level of benefits under the Pacific Blue Cross Plans.

  • Health Promotion and Health Education Both parties to this Agreement recognize the value and importance of health promotion and health education programs. Such programs can assist employees and their dependents to maintain and enhance their health, and to make appropriate use of the health care system. To work toward these goals:

  • Health Leave Any regular employee of the District may, at the discretion of the Board, be granted a leave of absence without pay for reasons of health, such leave to be specified for a period of not more than one year. Such leave may be extended in case of serious health conditions.
