Health Centre. The parties hereby agree, subject to the approval of the Ministry of Labour, that extended will be implemented under the following and conditions. In all other respects the collective agreement shall apply. All eligible full-time and regular part-time staff on a that is considering extended shift schedules will be given an opportunity to vote on the proposed schedule and shift rotations. The parties will jointly supervise such vote, which shall be held by secret ballot. Where of those employees eligible to vote have voted in favour of extended shifts, the new schedule will be implemented on a six-month trial basis and will be reviewed by both parties. This Model Agreement shall form part of the Collective Agreement between the parties herein, and shall apply to the described in Article of this agreement.
Health Centre. The Health Centre is equipped to handle most medical emergencies and provides a valuable resource for pre- ventative health care. If employees are experiencing any medical problems during their regular or overtime shift, whether occupa- tional or non-occupational, the employee must report to the Health Centre for assessment. If it is necessary to leave the plant, employees must get authorization to do so from their Supervisor, or if unavailable, from anoth- er member of management. In an emergency situation, leaving the plant requires appropriate notification (Team Leader, Supervisor, Security, Health Centre or Reception). In situations employees must inform their Team Leader or prior to reporting to the Health Centre. Personal Phone Calls Pay telephones are provided in the plant area for the employee's use in making outside local calls during lunch break or rest periods. Emergency incoming calls will be directed to employees immediately. The plant phone number is Phone calls during work periods should be for emergency purposes only. Cell phone use in the plant will only be permitted dur- ing lunch and/or break periods. Due to proprietary, personal privacy, and safety con- cerns video cell phones and cameras are not permitted in the Plant, unless authorized by the Company for busi- ness use.
Health Centre. An employee may have her hours of work averaged over a multi week schedule but will not be required to work more than four (4) consecutive extended tours. Premium pay as outlined in Clause will be paid for time worked on a fifth and subsequent extended tour. Schedules will be posted at least four (4) weeks in advance. The Hospital will schedule shifts such that there will be a minimum of twelve (12) hours off between shifts. The Hospital undertakes to schedule work to permit employees to receive two (2) weekends off in six (6). A weekend is defined as at least sixty (60) consecutive hours off work from the completion of the Friday shift until the beginning of the Monday shift. an employee is scheduled to be off over the Christmas period, her schedule shall be drawn up to include Christmas Eve (evening) off. The same shall apply for New Years. When a tour schedule is changed by the Hospital without four (24) hours notice, the employee shall be paid at the premium rate outlined in Clause for the first shift of the new schedule, This Clause should not be interpreted as applying to the cancellations resulting from decreased census or a change in shift at the request of an employee.

  • Health Care The Company will reimburse the Executive for the cost of maintaining continuing health coverage under COBRA for a period of no more than 12 months following the date of termination, less the amount the Executive is expected to pay as a regular employee premium for such coverage. Prior to January 1, 2020, bargaining unit employees with less than one (1) year of service will be provided coverage through the “Medical Only” plan.   • Occupational Health & Safety (a) It is a mutual interest of the parties to promote health and safety in workplaces and to prevent and reduce the occurrence of workplace injuries and occupational diseases.   • HEALTH & WELFARE 16:1 The parties signatory hereto shall enter into a Health and Welfare Plan for which there is a Trust Agreement, known as the Line Construction Benefit Fund, for the purpose of providing insurance benefits for eligible employees and/or their dependents. Effective the first of the month following the signature date of this Agreement, the Employer shall pay to the Line Construction Benefit Fund the sum of $6.50 for each hour worked. Hours worked shall be deemed to include straight-time hours worked, overtime hours worked, and report time not worked. Remittance shall be forwarded to the place designated by the parties hereto on or before the fifteenth (15th) day of each month for each hour worked in weekly payroll periods ending during the preceding month, together with a monthly payroll report on a form to be furnished to the Employer. It is understood and intended by the parties to this Agreement that the purpose of this clause is to establish an Employer financed Health and Welfare Trust and that contributions thereto shall not be deemed to be wages to which any employee shall have any right other than the right to have such contributions paid over to the Trust fund in accordance herewith. Failure of an individual Employer to make all payments provided for, including liquidated damages for late payments, within the time specified, shall be a breach of this Agreement and will further require action by the Trustees as set forth in the Trust Agreement. Any increase in the required contributions set forth above will be paid equally (50% by the Employer and 50% by the Employee). The amount paid by the Employee will come from their NEAP contribution.
