Health Benefit Plans. A. Upon ratification the employer health benefits contribution for each employee shall be an amount equal to eighty percent (80%) of the weighted average of the Basic health benefit plan premiums for a State active civil service employee enrolled for self-alone, during the benefit year to which the formula is applied, for the four Basic health benefit plans that had the largest State active civil service enrollment, excluding family members, during the previous benefit year. For each employee with enrolled family members, the employer shall contribute an additional eighty percent (80%) of the weighted average of the additional premiums required for enrollment of those family members, during the benefit year to which the formula is applied, in the four Basic health benefit plans that had the largest State active civil service enrollment, excluding family members, during the previous year. To be eligible for this contribution, an employee must positively enroll in a health plan administered or approved by CalPERS.
Health Benefit Plans. The College agrees to provide the following health benefit plans to all full time, probationary, continuous appointment and short-term appointment employees provided they meet the eligibility requirements of the Plans. Health Benefit Plans. .01 The Employer will use the best efforts to maintain the overall value of the Extended Health & Dental Benefits plan (Sun Life Financial Contract 4088, effective January 1, 2013) during the period of this Agreement. The Employer agrees not to amend the plans or premium cost sharing formula unless mutually agreed to by the Union and the Employer.
Health Benefit Plans. The health and welfare benefits outlined in this Article apply to all unit members, including those hourly unit members teaching in the Alternative Programs and/or Schools.
Health Benefit Plans. 1. Effective July 1, 2017, through December 31, 2018, the District shall offer the following healthcare options for employees:

Health Benefit Plans a) full-time employees and seasonal employees will be eligible for coverage as of date of hire; part-time employees will be eligible for coverage as of date of hire provided they work more the one-third of the normal hours for their classification; and term employees will be eligible for coverage after six (6) months of continuous employment. The Employer shall provide benefits, which shall be no less than those provided in Sun Life contract Number on February including:
Health Benefit Plans. Western shall provide faculty members with a preferred provider organization (PPO) health benefit plan. The “WMU Health Services Plan” as described in this article shall continue for the length of this Agreement. Western retains the right to determine carriers and administrative agents of such benefits and programs at its sole discretion, with the prior consultation of the WMU Chapter of the AAUP. Upon request, Western will provide the Chapter with a copy of all contracts with health care agents and providers. Human Resources shall assist faculty with questions and concerns regarding these health care plans.
Health Benefit Plans. 37.01 Full-time employees will be eligible for coverage following the completion of six (6) months service.
