HEALTH AND SOCIAL SERVICES. Upon the effective date of reversion, Martinsville shall no longer be responsible for the governance or funding of the Xxxxx-Martinsville Health Department or the Xxxxx-Martinsville Department of Social Services. Martinsville shall convey all its interests in the real property presently used by the Xxxxx-Martinsville Health Department (located at 000 Xxxxxxxxxxxx Xxxxxxxxx) and the Xxxxx-Martinsville Department of Social Services (located at 00 Xxxxxxxx Xxxxx) and all its interests in the personal property of the Xxxxx- Martinsville Health Department and the Xxxxx-Martinsville Department of Social Services to Xxxxx County upon the effective date of reversion. The October 1, 2011, Facility Costs Agreement among Martinsville, Xxxxx County, and the Xxxxx-Martinsville Department of Social Services shall be dissolved as of the effective date of reversion.

HEALTH AND SOCIAL SERVICES i. The Tribe will support local alcohol and other drug prevention and treatment policies, programs and community efforts. On the first business day of July each year, beginning with the first July after Opening Day, the Tribe will make Additional Contributions of $100,000 annually to Sacramento County drug and alcohol treatment programs and $100,000 annually to Sacramento County domestic violence programs for the purpose of providing improved social services to residents of Sacramento County. Beginning in year two (2), these payments shall increase at a rate of 2% annually.

HEALTH AND SOCIAL SERVICES. The parties share the view that young adults have a range of particular health-related needs. The parties agree to address these needs through the following priority actions: • Combating substance misuse through a concerted focus on supply reduction, prevention, treatment and research. Specifically, rehabilitation is being added as a fifth pillar of the National Drugs Strategy, in line with a commitment given in the mid-term review of that Strategy, and a Working Group is developing an integrated rehabilitation provision. Also in line with the mid-term review, the Department of Health & Children is exploring the potential for better co-ordination between the areas of drugs and alcohol with the aim of improving synergies. Additional funding is being provided in 2006 to develop facilities and services for young people. • Ensuring a greater focus on reducing alcohol related harm including implementation of the recommendations of the Working Group on Alcohol, established under Sustaining Progress, taking account of the recommendations of the Strategic Task Force on Alcohol. • Ensuring implementation of the National Strategy for Action on Suicide Prevention, 2005-2014. Specifically this will involve: o Working intensively to co-ordinate suicide prevention activities across the country. o Disseminating research and best practice and, where necessary, commissioning and supporting new research. o Consulting with those working to reduce suicide and those responding to suicide, to ensure their voice is heard in planning future suicide prevention initiatives.

The terms of this agreement govern the submission, by the Billing Agent, of the Provider’s clinical and financial information sent in support of services performed by the Provider. I, , as Billing Agent, enter into this Billing Agent Information Submission Agreement with the State as authorization to submit, in either electronic or paper format, the Provider’s clinical and financial information directly to the State on behalf of the Provider. All information submitted under the terms of this agreement is in support of services performed by the Provider.
