Health and Dental. Beginning August 1, 2007 previously employed teachers on less than a full-time contract shall be offered an additional enrollment period of 30 days to enroll in the GCS health insurance program. The previously employed teacher must enroll within the first 30 days of their first full time contract. Beginning January 1, 2008, the school corporation shall offer a Health Saving Account (HSA) health insurance option (Plan C HSA) and beginning January 1, 2009, new HSA enrollees shall have one half of either $1,000 (single) or $2,500 (family) deposited into the employee's HSA account in January and then the remainder of the amount will be divided into equal payments and deposited monthly beginning in July. After the first year of enrollment, GCS shall deposit monthly the yearly HSA amount divided by 12 into the employee's HSA. These amounts shall be prorated if the employee retires or resigns prior to contract completion or works less than full time. Employees electing Plan C -HSA in 2010 shall continue in this plan. After January 1, 2014, new HSA enrollees shall have their monthly contributions into the employee's HSA account determined by the insurance committee with the understanding that it shall not be less than the amount delineated above or the amount during the first year of employment delineated in the next paragraph. Beginning· on October 1, 2016, all newly hired employees hired after that date and current employees presently not enrolled in any insurance plan but due to a "life event change" become eligible for coverage will only be able to enroll in the Health Saving Account (HSA) health insurance policy. During the first year of employment the school corporation will contribute the first installment of employee's annual HSA amount (first year only amount - $1,250 single, $2,750 family; remainder of years the amount will be delineated in insurance plan) in the first month of the employee's enrollment in the plan. Health and Dental. Seasonal Employees who work six (6) months/(26 weeks) or more shall participate in the Health and Dental Care Plans of the Employer, cost shared at a rate of 65% Employer 35% Seasonal Employee, while the Seasonal Employee is actively at work. Seasonal Employees participating in these plans shall pay the full premium costs (both Employer and Employee share) for the months they are on seasonal lay-off. Seasonal Employees shall pre-pay premiums for the layoff period through payroll deductions which will be pro-rated biweekly prior to the date of layoff.

Health and Dental. (a) A group hospital, surgical, major medical and dental plan as agreed to by the parties shall be available to employees. In the event the Employer shall propose a change in this plan, this Contract shall be reopened for purposes of negotiations on such a proposed change. For the Point of Service (POS) plan, employees shall pay twenty five percent (25%) of the difference of the single or family HMO and POS premiums. Employees with a spouse on Medicare Plus will receive a payment not to exceed that paid by the Employer for family coverage. The Employer agrees that employees and their dependents selecting a health care provider offering Dane County both the Point of Enrollment/Point of Service and HMO plans will be allowed one (1) thirty (30) day enrollment period per year during which time an employee enrolled in the plan specified above can choose between that provider’s XXX/POS or HMO. For group dental insurance for 2021 the Employer shall pay up to forty-eight dollars and ninety-four cents ($48.94) for employees desiring the "single plan", up to one hundred thirty-eight dollars and two cents ($138.02) per month for those desiring the "family plan". For the first four (4) years of such retirement, the Employer will pay a maximum of ninety dollars ($90.00) per month toward the health and/or dental premium cost of the retired employee, surviving spouse and any eligible dependents, with the Employer applying any accrued sick leave credits of the retired employee as provided in Article XV, Section 6 (a), to any excess cost over the ninety dollars, ($90.00) per month. Retired employees, without sick leave credits, may pay this difference or full cost of premium or subscription after four (4) years of retirement and continue in either plan to age sixty-five (65) by making payments of the difference or full cost to the Employee prior to the tenth day of the month preceding the month of coverage. [The provisions of this paragraph may be delayed for the same period as detailed in Section 15.06(a)].

Health and Dental. The Employer currently does not have a health and dental plan, however, when the Employer is in a position to have such a plan, the Executive will be offered the opportunity to participate in the plan.
Health and Dental. The Employer will provide nurses covered under this contract with the same group insurance providers and benefits, at the same nurse [and dependent, if applicable] cost- sharing as the hourly non-bargaining unit employees at the unit have, including medical, dental, life, disability insurance. No dispute with regard to any plan shall be subject to the grievance or arbitration process provided in this Agreement.

Notwithstanding any of the above, if any “buy- up options” to reduce deductibles, coinsurance, or other out-of-pocket amounts are offered for the plan in which the employee has elected to participate, and the employee elects such options, the company will maintain current practice based on plan design.
