Group Medical Benefits. Group medical, dental and vision insurance when they become benefits eligible, which is on the first of the month following ninety (90) days of continuous employment. The Company shall contribute the amount required per hour under the HCSO to Group Medical Benefits for each eligible employee. The Company shall contribute the following amounts toward the cost of group health insurance including medical, dental and vision:
Group Medical Benefits. Under the terms of the Flexible Benefit Plan, an employee who enrolls in Freshwater’s group medical plan will automatically pay his/her share of the premiums for that coverage (if any) on a pre-tax basis through the Flexible Benefit Plan. An employee who has enrolled in the group medical plan and wishes not to pay premiums on a pre-tax basis must affirmatively elect to pay the premiums with after-tax salary deductions by completing and submitting a separate form, which is available upon request. Other Benefits I elect to receive the following reimbursements for the plan year.
Group Medical Benefits. Xxxxxxxxx shall be entitled to continue any existing group medical benefits he may have elected under Section 5(c) of the Consulting Agreement, subject to the terms thereof.

Xxxxxxxxx shall be entitled to receive continuation coverage in Sapient's medical and dental plans pursuant to the provisions of the Consolidated Omnibus Budget Reconciliation Act of 1985 ("COBRA"), such continuation coverage shall be at Xxxxxxxxx'x expense, and in all respects be subject to the requirements, conditions and limitations of COBRA and such plans, which may be amended from time to time.
Group Medical Benefits. (a) Effective January 1, 2018, the University shall provide and maintain the following medical benefits for each employee regularly assigned to work twenty (20) hours or more per week, commencing with the employee’s ninety-first (91st) day of service.

Hacienda, at its expense, shall provide for Executive full family medical and dental insurance as well as accident and health, and income continuation insurance benefits (including disability) for Executive equivalent to the normal and customary benefits available from time to time in accordance with the policies of HOB for itself and its subsidiaries for an employee of Executive’s salary level
Group Medical Benefits. The COMPANY agrees to provide health Insurance through Family Medical Care Plan (FMCP) through December 31, 2018.
Group Medical Benefits. Greenberg shall be entitled to continue any existing group medical bxxxxxxx xe may have elected under Section 5(c) of the Initial Consulting Agreement, subject to the terms thereof.
Group Medical Benefits. 23.01 The Employer agrees to pay as of January 1, 2014, seventy (70%) percent of the total premium cost for the Encon Group Inc. Insurance plan and the Employee agrees to pay thirty percent (30%) of the total premiums. The plan includes an extended health and a dental plan for all regular employees, including eighty per cent (80%) basic dental and fifty percent (50%) major dental coverage. Vision Care amount remains at three hundred dollars ($300.00) and Dental coverage remains at two thousand five hundred dollars ($2500.00). An Employee Assistance program for regular full time Employees and regular part time Employees who have completed the probationary period is also provided.
Group Medical Benefits. You will continue to receive coverage under the provisions of the Royal Appliance Group Medical Coverage Plan through Royal paying the COBRA through August 15, 2003, or until benefits become effective at another employer, whichever comes first, on the same terms and conditions as presently provided by Royal Appliance. At that time, you will be entitled under COBRA to continue coverage for up to an additional 6 months provided that you pay the cost of the premium at the time that you elect to continue coverage. Further information regarding your COBRA rights will be sent to you at a later date.
