General Policies. 1. The District, at any time, may require adequate confirmation of stated reasons for leaves. For six (6) or more consecutive days of personal illness/extended illness a Report of Extended Leave/Accident form will be required. However, a doctor’s note is sufficient in lieu of the Report of Extended Leave/Accident form if it contains all of the necessary information listed on the form.
