The certificated employee or the evaluator may withdraw consent at any time. Both the evaluator and the certificated employee must provide written consent annually and note the year of the evaluation cycle.\n7.2.4 No later than the end of the seventh school week of the year, the evaluator and the certificated employee shall meet and mutually agree to instructional goals and objectives for the school year upon which evaluations of the employee are to be based.\n7.2.5 During the course of the evaluation period, circumstances may arise which require modification of the evaluation process. Such modifications shall be accomplished by mutual written agreement between the evaluator and the evaluatee.\n7.2.6 The formal written evaluation of a teacher shall be based upon a number of classroom observations of sufficient length to enable the evaluator to form a valid opinion of the teacher's work.\n7.2.6.1 Lesson observation(s) for the purpose of formal evaluation, shall normally be arranged by the evaluator and the evaluatee by mutual agreement at least 48 hours in advance of the observations. Short-term observations may be carried out on an unannounced basis.\n7.2.6.2 Any certificated employee who receives an unsatisfactory observation may, upon request, be entitled to a subsequent observation, conference and written evaluation, as prescribed above. Permanent employees who receive an unsatisfactory evaluation may request a different qualified evaluator prior to a mandatory annual evaluation.\n7.2.6.3 The formal filing of the evaluation shall be preceded by an evaluation conference in which the evaluator and the certificated employee shall review the observation(s) and what is to be incorporated into that evaluation report within five working days.\n7.2.7 An evaluator shall base a formal evaluation of a certificated employee on information which is collected through direct observation of such employee or other documented information. This excludes public charges and other negative material in the personnel file that has not been documented.\n7.2.7.1 Permanent certificated teachers who receive an unsatisfactory evaluation in the areas cited in Article 18 shall be referred to Peer Assistance and Review.\n7.2.8 The certificated employee's evaluator shall take affirmative action to assist the teacher in the correction of any cited deficiencies. Such action shall include specific recommendations for improvement and direct assistance in implementing such recommendations.\n7.2.8.1 Permanent certificated teachers who receive an unsatisfactory evaluation in the areas cited in Article 18 shall be referred to Peer Assistance and Review.\n7.2.9 If subsequent remedial action eliminates the negative evaluation and/or the identified deficiencies, the evaluator shall place appropriate comments in the evaluatee's personnel file.\n7.2.10 Teachers shall not be required to participate in the observation and/or evaluation of other teachers other than within the formal structure of PAR. Each tenured certified staff member shall be evaluated at least once every two years. Non-tenure certified staff members shall be evaluated once or more each year. The order of evaluation will be at the principal\u2019s discretion.\nB. All monitoring or observation of the work of each employee shall be conducted in person and any additional monitoring or input from other persons shall be with full knowledge of the certified staff member being evaluated.\nC. A mandatory summative conference will be held within ten (10) school days of the final observation, unless by mutual agreement more time is needed.\nD. The evaluation sheet will be reviewed jointly by the certified staff member and the principal. Employees who have floating or multiple building locations shall be assigned a single supervisor for purpose of evaluation.\nE. A certified staff member may submit additional written comments to the written evaluation within 30 days of receipt of the written evaluation if he/she so desires. All written evaluations and the certified staff member\u2019s comments are to be placed in the certified staff member\u2019s personnel file.\nF. The certified staff member will receive a copy of the evaluation and all recommendations made by the administrator which are to be placed in the certified staff member\u2019s personnel file.\nG. During the summative evaluation conference, the certified staff member will sign the evaluation report. In no case shall the certified staff member\u2019s signature be construed to mean that he/she necessarily agrees with the contents of the evaluation.\nH. The Administration will strive to uniformly apply the procedures of evaluations throughout the District. Problem areas of uniformity may be directed to the joint committee.\nI. Evaluation procedures:\n1. Within one month after the beginning of the fall term, the building principal shall inform each certified staff member to be evaluated that school year of the evaluation procedures, standards and instruments to be used and each certified staff member\u2019s right of response to the principal\u2019s evaluation.\n2.
