Filing Application. Unless there are extenuating circumstances, the employee shall submit the required application for sick leave pay within sixteen (16) working hours after their return to duty. However, if the employee is absent because of illness or injury for more than eighty (80) working hours, they shall then file an application for Family Medical Leave. Each supervisor shall obtain the necessary forms provided by the Seattle Human Resources department and make them available to the employee.
