Feedback Reports. Generate feedback reports which contain data on patient demographics and variations in patterns of care. By virtue of this approach, each site is able to compare their performance with other sites and the consortium as a whole in a non•threatening confidential manner. Review of these data will focus on identification of variations in care and outcomes across sites, in order to identify high-yield opportunities for quality improvement. In particular, these reports are used to identify process of care variables that may correlate with outcomes, as well as to uncover and address local barriers to optimizing the implementation of evidence-based practice patterns.
Feedback Reports. These files will include summary data aggregated to the MDPCP Practice Site level on data relevant to MDPCP Practice Site Activities and performance in the MDPCP, including the demographic and claims-based data described above. When aggregated at the MDPCP Practice Site level, this aggregate information will not include individually identifiable information and will incorporate de-identified data from MDPCP Beneficiaries who have opted out of data sharing or opted into data sharing pursuant to Articles 10.4 and 10.5. These reports are provided for informational purposes.
Feedback Reports. Distributor will submit to Company, upon receipt of such information and upon request by Company, any competitive information, problems, complaints or suspected defects with respect to the Product, and any feedback from any Customer.
