Extended Health. The Board shall pay fifty (50%) of the premium cost of the Provincial Extended Health Benefit Plan for each full-time and part-time teacher employed by the Board. The plan will currently include the following: i. supplemental travel rider payable at one hundred percent (100%) reimbursement for the following Medical Travel Expenses: (1) Transportation for a member and/or dependents by scheduled air, rail, ferry or bus to and from the nearest locale equipped to provide the treatment required, when ordered by the attending physician and surgeon because, in their opinion, adequate treatment is not available locally. Said transportation must be within two (2) months of referral and will not be provided to points beyond Vancouver, B.C. (2) Transportation of an attendant for the patient being transported under B.11.3.b.iii.1 above, when ordered by the attending physician and surgeon. (3) Accommodation in a commercial facility for the patient only, before and after medical treatment to a maximum of $45/day for a total of seven days. Accommodation is not provided for the attendant.
