Client Population Adult individuals aged 16-65 with the diagnosis of schizophrenia and schizophrenia spectrum disorders who could benefit from support with optimizing their recovery based on their goals. HPS clients reflect a wide variety of cultures. HPS provides services in English. HPS is designated and identified as a French Language Service. When requested or needed, interpretation and translation support is accessed (i.e. SISO). Expected client outcomes: all referred clients are supported for as long as they continue to require assistance without barriers based on culture, French language or Aboriginal status. Geography Served HPS's catchment includes Area 7 - East Hamilton and Area 8 - West Hamilton. However, HPS presently supports clients from Area 4 - Burlington, Area 5 - Grimsby, Area 6 - Pelham and Niagara Falls and LHIN 6 - Xxxxxx.
