Examination and Treatment. The CONTRACTOR shall have written policies and procedures to assure dental screenings, exams, x-rays, and treatment are rendered consistent with the CDCR and ACA standards. The CDCR records sent to the facility will be reviewed for dental history and to identify current dental care that should be continued.

Examination and Treatment. I understand that I may require medications, examinations, diagnostic procedures or other treatments in connection with my condition. I further understand that the hospital may use for diagnostic, education, quality improvement, scientific or certain research purposes or may otherwise dispose of, tissue and biologic fluids such as blood and urine, removed during such procedures. I hereby consent to the performance of such examinations, treatments, and procedures, as appropriate personnel deem necessary or advisable. I understand that the practice of medicine and surgery is not an exact science, and I acknowledge that no guarantees have been made to me as to the results of treatments or examinations in the hospital. I understand that I have the right to withhold consent to any medical or surgical procedure. I understand that the hospital has the right to decline to permit the performance of any procedure if there is not satisfactory assurance that informed consent was given. I realize that if I withhold consent for a recommended procedure that treatment may be rendered partially or wholly ineffective. I understand that physicians on the staff at the Lifespan hospital/affiliate may not be employees of the Lifespan hospital/affiliate. MEDICAL EDUCATION I understand that during my care, I may be examined and treated by physicians and other personnel in training under supervision. PHOTOGRAPHS, VIDEOTAPES, DIGITAL OR OTHER IMAGES I understand that photographs, videotapes, digital or other images may be taken for identification purposes or to document my medical condition or care.

  • Health Examinations The Employer shall provide at no cost to the employee, such medical tests, health examinations and surveillance/monitoring as may be required as a condition of employment and/or as a result of regulated hazards encountered after employment.

  • Health Examination 27-1 When the District determines that a MBU's health condition (mental or physical) may be impairing his/her job performance, the immediate supervisor, site administrator, or Regional Assistant Superintendent, with the concurrence of the Human Resources Department may, with just cause, direct the MBU to have a health examination at District expense.   • Medical Examinations An employee may be required by the Employer, at the request of and at the expense of the Employer, to take a medical examination by a physician of the employee's choice. Employees may be required to take skin tests, x-ray examination, vaccination, inoculation and other immunization (with the exception of a rubella vaccination when the employee is of the opinion that a pregnancy is possible), unless the employee's physician has advised in writing that such a procedure may have an adverse affect on the employee's health.

  • Examination and Review A volunteer, upon presenting identification, shall be permitted by appointment to examine and copy his/her complete County volunteer file, MCFRS operating, or medical record. The volunteer shall indicate in writing, to be placed in his/her file, that he/she has examined the same. The custodian of medical records may determine, consistent with State law, that certain medical information will only be released to the physician or attorney of the volunteer upon receipt of a signed release from the volunteer.
