Evaluation Reports. Performance evaluations are not part of the disciplinary process. Where a formal evaluation of an employee's performance is carried out, the employee shall be provided with a copy to read and review. At the end of the performance appraisal meeting, the employee shall sign the form indicating that the performance appraisal meeting took place with the manager. The employee shall receive a copy of the evaluation report at the time of signing. The employee has the right, within seven (7) calendar days, to provide any written response or comments to the performance appraisal. This response must be signed and dated by the employee and shall be appended to the evaluation and maintained on the employee’s personnel file. An evaluation report shall not be changed after an employee has signed it, without the knowledge of the employee.
