Eligibility for Service. The Extended Maintenance Agreement shall only be valid as long as the equipment covered by it is properly installed, and is serviced by EndoSonics authorized personnel only. EndoSonics site environmental conditions must be met at all times.

Eligibility for Service. You understand and agree that you must meet and maintain our qualifications in order to use the Service. You further understand that we use an automatic enrollment process to determine eligibility to use the Service and that you may lose access to the Service, either temporarily or permanently, if you do not meet our qualifications. If you do not meet our qualifications, you agree that you will not be able to use the Service and will need to mail in deposits, utilize an ATM, or bring checks to a branch location for deposit. In addition to other qualifications, you understand you must have a membership open for a minimum of 30 days and must not be in delinquency or default on any loans or accounts at Empower Federal Credit Union. Additional qualifications: • Must be at least 18 years of age • Must have both a share savings account with par value in it and a qualifying checking/share draft account: regular share draft, Premium Money Market with checks, Basic Business Checking, Business Checking with Dividends, Share Draft Checking Plus, HSA Checking, Business Money Market with checks, Business checking com core, Com Core Checking. You understand that your ability to utilize the Service may be limited or terminated at any time and that we do not need to notify you before taking such action. Guarantee Specific to Deposits Received for Credit to a Consumer or Business Account: Your use of the Service for the purpose of depositing to a consumer or business account constitutes the understanding and agreement that you are personally liable for any expenses Empower Federal Credit Union incurs in attempting to obtain final payment for the item in question, outside of the routine costs associated with item processing, in the event of a default by you and/or by your business. This includes, but is not limited to: (i) recovery of the amount credited in the event of non-payment, collection costs, and attorneys’ fees as applicable; and (ii) any and all costs associated with Empower enforcing this guarantee. This guarantee shall benefit Empower and its successors. Eligibility for Service. Commencing on the Effective Date of this Agreement, the Parties agree that only properties located within the corporate boundaries of both Parties will be eligible for the Offering of Service and/or Provision of Service in the Area through the Joint Assets, unless otherwise mutually agreed upon by the Parties.
Eligibility for Service. Good may implement further integration with your Mobile Carrier allowing your data usage via the Apps to be paid directly without appearing on your monthly Mobile Carrier invoice. This may result in you no longer being able to use the Service to receive a monthly Reimbursement. If this happens, then Good will notify you that you will no longer be eligible to receive reimbursements through the Service. In addition, Good may suspend or terminate your access to the Service for any or no reason without liability to you.
Eligibility for Service. Subrecipient may determine eligibility for all service recipients directly or through a subcontract or other agreement with LCDPD or a public or private nonprofit agency or organization. The eligibility of individuals to receive purchased services shall be determined in accordance with the policy and procedures established by the Xxxxx County Workforce Development Board (LCWDB) in accordance with state, federal, and local guidelines governing eligibility for WIOA services. The LCDPD may make eligibility determinations if it concludes that doing so would (i) benefit clients; or, (ii) result in more efficient operation of social service programs in Xxxxx County. The LCDPD may also make eligibility determinations. Subrecipient shall maintain all necessary documents demonstrating a proper eligibility determination was made for each and every eligible individual. Such records shall be subject to inspection, review, or audit by duly authorized federal, state, and LCDPD personnel at all reasonable times. Such records shall also be subject to inspection by the individual or entity selected for the audit required by Article 11 of this Contract. Eligibility records shall be kept by Subrecipient in accordance with the provisions of Article 23, herein.
Eligibility for Service. Microsoft reserves the right to change the products supported under a Services Description during the term of the Services Description. Microsoft may discontinue support for a product, provided that AGENCY will be given six (6) months notice of that discontinuance. As an exception to the foregoing, should support for a product be discontinued by Microsoft because it has been sold to another company, Microsoft will provide written notice of the sale to AGENCY and, at Microsoft's option and in its sole discretion, either arrange for the acquiring company to begin providing support for that product immediately upon the date of notice, or provide continued support for that product for ninety (90) days from the date of notice. Microsoft may add support for new products during the term of a Services Description pursuant to the terms and conditions of this Agreement. On a monthly basis, items or services may be added or deleted by OGS to this Master Agreement pursuant to the standard Agreement amendment procedures set forth in Appendix B. These procedures shall be required for changes in Microsoft's service delivery programs; but shall not be required for any amendments to the list of products supported by Microsoft under the existing Premier Services Description pricing plans or the execution of any Work Order or Services Description. Any credits due under this Agreement, whether from billing adjustments, changes in coverage, price reductions, penalties, or any other reason, shall be in the form of Credit Memorandum issued by Microsoft to AGENCY. AGENCY may, at its sole discretion, apply the Credit Memorandum amount towards any service offered under this Agreement. Under such circumstances, when the application of credit has no value to the Agency, such credit shall be paid in cash to the Agency .
Eligibility for Service. City shall determine the eligibility of the residents for such service and shall have plenary discretion over eligibility criteria. No resident of the City shall be eligible unless he or she is a disabled ambulatory resident, as defined by current eligibility requirements. Once approved by City, residents shall be issued a photo identification membership card. Contractor shall not be required to render service to any resident of City unless he or she has been issued a photo identification membership card.

Eligibility for Service. To use this Service, you must adhere to the following: Have a Credit Union membership account in good standing (good standing to be determined by the Credit Union in its sole and absolute discretion). Have a qualifying membership account. Be registered to use AmeriCU's Online Service.
Eligibility for Service. This Customer Agreement describes Terms & Conditions for all available Plans. Plans are available in all the areas that the CBPU serves. The enrollment confirmation indicates the individual Plan(s) in which the Customer has enrolled. The Customer can also confirm active enrollment in individual Plan(s) by calling the CBPU. In order to be covered by a Plan, all equipment must: (a) be installed to meet local, state and federal codes; (b) satisfy manufacturer requirements for safe and proper operation; and (c) be in good working condition at the time of enrollment. By permitting the Customer to enroll in a Plan, the CBPU does not make any express or implied warranties concerning Customer’s existing equipment or conditions. The CBPU may refuse to provide service or deny enrollment under the Plan if eligibility requirements are not met. At its discretion, the CBPU reserves the right to inspect the lines and deny enrollment in a Plan. The Plan will begin after the first xxxx, after receipt of the first payment, or 15 days after enrollment (whichever is later). The Plan is suspended at the end of the prepaid period in the event of nonpayment. The Plan must be current in order for the Customer to receive service under the Plan. If the Plan is suspended for nonpayment, the Customer’s participation in the Plan will resume 15 days after payment is received. Any repairs that may be needed prior to the effective date or while the Plan is suspended for nonpayment are not covered by the Plan.
Eligibility for Service. The Company shall determine the eligibility of Signal Hill residents for Dial-A-Lift services and issue a Dial-A-Lift Membership Card to those certified as eligible.
