Disability Insurance Premiums. The Corporation may, at its option, provide Executive with the opportunity to elect to include the amount of any disability insurance premiums paid by the Corporation pursuant to any disability insurance, plan, or policy provided by the Corporation to or for the benefit of Executive as taxable income to Executive. If Executive so elects, the Corporation shall pay to Executive an additional amount necessary to put Executive in substantially the same after-tax position that he would have been in had he not elected to include such disability insurance premiums in income (taking into account all federal, state, and local income and employment taxes due as a result of the inclusion of such disability insurance premiums in income). Payment of the additional amount, if any, shall be made to Executive in the same pay periods in which the disability insurance premiums are included in income.

Disability Insurance Premiums. Allos shall reimburse Dr. Gerber, on a grossed-up basis, for the premixx xxxxx xx incurs in continuing in effect the supplemental disability insurance policy under which he is now insured for a period of 24-months following the Separation Date.
Disability Insurance Premiums. During the EMPLOYMENT TERM, DMC shall pay the annual premiums necessary to maintain a disability insurance policy in favor of the EXECUTIVE, issued by a company chosen by DMC and reasonably acceptable to EXECUTIVE, which would provide aggregate annual payments to EXECUTIVE or such other beneficiary as may be designated by EXECUTIVE, of One Hundred and Thirty Thousand Dollars ($130,000).
Disability Insurance Premiums. Executive and the Corporation acknowledge that there has heretofore been issued and is presently in effect a non-cancelable guaranteed renewable disability income policy covering Executive issued by The New England Mutual Life Insurance Company of Massachusetts, Policy No. 191D2779000, which policy provides for a maximum monthly benefit of $8,000 until age 67 payable to Executive. Corporation agrees to make the premium payments with respect to such policy to The New England Life Insurance Company of Massachusetts on Executive's behalf as they become due.

  • Life and Disability Insurance The Company will provide term life and disability insurance payable to the Employee, in each case in an amount up to a maximum of one times the Employee’s base salary in effect from time to time, provided however, that such amount will be reduced by the amount of any life insurance or death or disability benefit coverage, as applicable, that is provided to the Employee under any other benefit plans or arrangements of the Company.   • Insurance Coverages The Design Professional will promptly obtain and maintain the insurance coverage described on the Schedule of Project Details and provide the Owner with proof of that insurance before commencing services under this Agreement. The Design Professional agrees that the Services may not commence until all proof of required insurance has been provided to and approved by the Owner.
