Disability and Death Benefits. If death occurs prior to the Life Insurance and Disability Income Benefit Plan of the United Church of Xxxxxx becoming effective, we will provide your family with no more than four months of cash salary, housing, Social Security and Medicare offset, and benefits, coordinating the provisions of the Disability Benefits and the Death Benefits described above.

Disability and Death Benefits. If the Executive dies or becomes disabled, during the term of this Agreement or during any consulting agreement entered into pursuant to this Agreement, the Company shall pay to the Executive, the Executive’s attorney in fact, estate or beneficiary, as appropriate, an amount equal to 100% of the Base Salary in effect at the time of death or disability, or termination of employment, if under Consulting Agreement. The payment shall be payable in monthly installments over a 12-month period.
Disability and Death Benefits. (a) If Employee becomes disabled prior to his retirement he shall be entitled to an annual disability benefit equal to fifty percent (50%) of his Annual Compensation Value; provided, however, that such annual benefit under this Agreement shall not exceed $500,000. The disability benefit shall be provided through the then existing Reynxxxx xxxnsored disability plan with Reynxxxx xxxing any additional contributions as may be necessary to pay Employee the required amount, and said benefit shall also be offset by any Retirement Benefits which Employee may then be receiving. The disability benefit, including any Reynxxxx xxxuired contribution, shall be paid so long as and on the same terms and conditions as the payments being made under the Reynxxxx xxxnsored disability plan. See Section 7(c) below. (b) In the event of Employee's death while still employed by Reynxxxx xxxsuant to this Agreement, Employee shall be entitled to Retirement Benefits calculated as if he had elected retirement as of the day before his actual death. Reynxxxx xxxll also pay to such beneficiary or beneficiaries as he shall have designated by written notice delivered to Reynxxxx xxxor to his death, or failing such written notice, to his estate, an amount equal to the Base Compensation plus the Bonuses, if any, which Employee would have received or which would have been accrued for his benefit during the period of six (6) months immediately following his death if he had lived and had been employed by Reynxxxx xxxing that period. Such payment shall be made in one lump sum or in six (6) equal monthly installments as Reynxxxx xxxll elect and shall be in addition to the proceeds of any insurance policies carried on Employee's life with respect to which he has the right to designate beneficiaries. Also, Reynxxxx xxxll pay to Employee's spouse an amount, periodically as such payments are required to be made by said spouse, to enable her to continue medical coverage for her and her dependents in the same manner as immediately prior to Employee's death for a period expiring at the earlier of: (i) her death; (ii) forty-two (42) months after Employee's death; or (iii) eligibility for regular Medicare and Medicaid or any successor programs furnished by the government. Thereafter, Reynxxxx xxxll make available to Employee's spouse (including her dependents), at her cost, such medical coverage as shall be available to a person of her then age under the then-existing Reynxxxx-xxxnsored medical benefits ...
Disability and Death Benefits. If during the term of this Agreement Employee either dies or becomes physically or mentally disabled (as determined in accordance with the Social Security Act) from performing his functions contemplated hereunder, and such period of disability continues for at least six consecutive months, then Employer will pay to Employee, or, in accordance with Section 16 below, his Representatives (as defined in Section 16(c)), as the case may be, the annual salary provided hereunder, together with the annual bonus above provided pro-rata, for a period through the end of the month in which Employee dies or the end of the month in which the six-month disability period is satisfied, whichever occurs first. Regular salary, pro rata bonus, and other payments, shall be made to Employee or Employee's Representatives, as the case may be, during the first six (6) months of any period of disability.

Disability and Death Benefits. A Participant who is permanently and totally disabled within the meaning of section 415(c)(3)(C)(i) of the Code for an accrual computation period shall receive a Year of Participation with respect to that period. In no event shall more than one Year of Participation be credited for any twelve (12) month period. The reduction described in this subsection shall not apply to disability benefits or death benefits as provided in the Code.

Such Special Risk Accident Policy now and in the future shall contain, at a minimum, a maximum accidental death benefit of Ten Thousand Dollars ($10,000) and, at a minimum, a maximum accidental dismemberment benefit in the amount of Ten Thousand Dollars ($10,000), provided such injury is sustained while actually on duty under the supervision of the policyholder (the Township) or while participating in fund raising drives, training classes, tests, drills, or trial of a piece of apparatus connected directly with such duties. All riders to the present existing policy shall be required and maintained by the Township, unless same are involuntarily canceled. A copy of the Special Risk Accident Policy is annexed hereto and made a part hereof.

Disability and Death Benefits. In the event Employee shall (i) become physically or mentally disabled (as determined in accordance with the Social Security Act) from performing his functions contemplated hereunder, and such period of disability shall continue for at least six consecutive months, or (ii) become deceased during the term hereof, Employer shall pay to Employee, or, in accordance with Section 16 below, his Representatives (as defined in Section 16(c)), as the case may be, the annual salary provided hereunder, together with the annual bonus above provided pro-rata, for a period through the balance of the month in which the described disability period begins, or the balance of the month in which the date of death, whichever shall occur sooner. Regular salary, pro rata bonus, and other payments, shall be made to Employee or Employee's Representatives, as the case may be, during the first six (6) months of any period of disability, physical or mental, as above described.
