Dental Records. Dental records concerning services rendered to Member shall remain the property of the Participating General Dentist or Participating Specialist. Member agrees that his/her dental records may be reviewed by Company as deemed necessary in compiling utilization and/or similar data. Company agrees to honor confidentiality of said data.

Dental Records. Service Company shall establish, monitor, and -------------- maintain procedures and policies for the timely creation, preparation, filing and retrieval of all dental records generated by Provider in connection with Provider's provision of Dental Care; and, subject to applicable law, shall ensure that dental records are promptly available to dentists and any other appropriate persons. All such dental records shall be retained and maintained in accordance with all applicable state and federal laws relating to the confidentiality and retention thereof. All dental records shall be and remain the property of Provider.
Dental Records. At PLAN’S reasonable request from time to time, SPECIALIST shall make available to PLAN or PLAN’S designee copies of dental records of Subscribers for review and/or copying. SPECIALIST agrees to obtain all proper releases from Subscribers needed under applicable federal and state law to comply with this request. PLAN agrees, and will require designee of PLAN to agree, not to disclose any patient identifying information obtained from the dental records of Subscribers. SPECIALIST shall retain all dental, financial, accounting, administrative and claim records of Subscribers for at least ten (10) years.
Dental Records. Dentist agrees to ensure that dental records are maintained for each Subscriber for whom the Dentist has provided services. Dentist shall allow the Plan or its designee, any state and/or federal regulatory agency, and any external review organization access to copies of dental records of Subscribers for quality assurance, investigation of complaints or grievances, enforcement of regulated activities or any other purpose related to compliance with state and federal laws and Plan’s Provider Manual and policies and procedures, subject to applicable laws related to confidentiality. Where applicable, Dentist agrees to obtain all proper releases from Subscribers needed under applicable state and federal law to comply with this request. Such records shall be retained and kept confidential by the Dentist for the greater of (a) the timer period required by applicable state and federal laws, and (b) ten (10) years. The obligations of Dentist under this section of the Agreement shall survive the termination of this Agreement. Notwithstanding the foregoing, Dentist shall retain all financial, accounting, administrative and claim records related to services provided to Subscribers for at least ten (10) years.

ORTHODONTIST shall retain all dental, financial, accounting, administrative and claim records of Subscribers for at least ten (10) years.
Dental Records. Participating Provider shall ensure that an accurate and complete patient dental record is established and maintained for each Enrollee. At a minimum, the record shall include all information about the Enrollee and a description of all services rendered to the Enrollee as dictated by generally accepted dental practice and standards and as required by the Contract.

Dental Records. By signing the document entitled ‘Essentials Contract’ you consent to the disclosure of your dental records for the purposes of any review, assessment or consideration of the care provided by your dentist which may take place under the terms of his or her membership of Denplan but not for any other purpose without your further consent.
