Dental and Vision Programs. A) Dental: The State will provide a dental plan for the employees and their family. The coverage shall be $1,200 through December31, 2018 . Effective January1, 2019, the State will provide a dental plan for employees and their families. The coverage shall be $1,500 per calendar year, in addition to the enhancements below. The State will offer benefit enhancements, including two buy-up options. Said modified plan enhancements shall include: • Add sealants as a preventive benefit for children under age14, covered at100% • Remove the $ 400 inside maximum for periodontal services • Increasethe lifetime maximum for orthodontic services from $ 850 to $1500 • Extend coverage to dependent children to age26 For the buy-up plans, the additional cost above the modified plan shall be paid for by the employees choosing the buy-up at 100% paid through increased premium co-shares.

Dental and Vision Programs. A Casual collector who wishes to participate in the AUTHORITY'S Dental and Vision Insurance Programs, as described in Article 8.2, may do so, provided that he or she contributes one hundred (100%) percent of the cost of such program on a monthly basis, payable no later than the first day of each month of coverage. Failure to make such payment in a timely manner shall void the coverage and, once voided, coverage shall not be renewable for a period of six (6) months.

  • Vision Plan The District will also make available a vision plan to be paid by the employee with pre-tax dollars through payroll deduction.

  • Vision Care Effective July 1, 2000, the District shall provide all full-time employees and their dependents with Vision Service Plan (VSP) Plan C. This plan shall provide for a comprehensive exam and new lenses every 12 months, and new frames every 12 months. All other services will be pursuant to the standard VSP plan description, except that it will reimburse up to $50 for examinations by non-panel providers. There shall be a $10 annual deductible on materials only. In addition, the following vision plan enhancements shall take place effective July 1, 2000: $60 wholesale frame allowance; computer glasses; progressive lenses, tints, and UV coatings.

  • Vision The University shall make available vision insurance to the staff members covered by this agreement to the same extent and in the same manner as is available to other University staff members, such as Faculty and the Executive, Administrative and Professional Staff members. It is the University's goal to have the same vision insurance plan(s) offered uniformly to all University staff member groups and staff members.

  • Dental specific medications for dental purposes, including fluoride medications (except for children less than five years of age with a non-fluorinated water supply);
