Unless otherwise agreed to, all deliveries ordered under this BPA must be accompanied by a delivery notice, ticket or sales slips that must contain at a minimum the following information: a. Vendor Name b. GSA Schedule Contract Number c. BPA Number d. Product Description/Model numbers e. Delivery Order/Reference Number f. Date of Purchase g. Quantity, unit price and extended price h. Delivery location and point of contact, with name and phone number i. Date of Shipment
