Declaration of Agreement. Should either Party request a Declaration of Agreement be executed for fling in the applicable public records, both Parties agree to execute a Declaration of Agreement similar in form as that which is attached hereto as Exhibit "G". Either Party will have the right and option, at its own expense, of fling such Declaration of record in the appropriate State and/or Federal offces.

Declaration of Agreement. This Agreement shall be effective upon the execution of a Declaration of Agreement by the Participating Provider and the Group wherein the terms and conditions of this Agreement are incorporated by reference. This Agreement, and the Declaration of Agreement, constitute the entire contract and all the terms and conditions agreed upon by the parties hereto regarding the subject matter of this Agreement. The parties accept the facsimile or reproduced signature of the persons executing said Declaration of Agreement on behalf of the Group and the WPPA as binding as if signed personally by them. Any prior agreements, promises, negotiations or representations, either oral or written, relating to the subject matter of this Agreement not expressly set forth in this Agreement are of no force or effect.
