Data; Reports. Provider shall cooperate with and release to Subcontractor and/or Health Plan any information necessary for Subcontractor and/or Health Plan to perform its obligations under the State Contract to the extent applicable to Provider in performance of the Agreement, including the timely submission of reports and information required by Subcontractor and/or Health Plan, in the format specified by Subcontractor, Health Plan and/or the State. Such reports shall include child health check-up reporting, if applicable, as well as complete and accurate encounter data in accordance with the requirements of Subcontractor and/or Health Plan and the State. Data must be provided at the frequency and level of detail specified by Subcontractor, Health Plan or the State. By submitting data to Subcontractor and/or Health Plan, Provider represents and attests to Subcontractor, Health Plan and the State that the data is accurate, complete and truthful, and upon Subcontractor’s and/or Health Plan’s request Provider shall certify in writing, that the data is accurate, complete, and truthful, based on Provider’s best knowledge, information and belief.

Data; Reports. Client owns the Data Reports. To the extent that any Intellectual Property Rights of Serraview are incorporated into the Data Reports, Serraview grants to Client a non-transferable, non- exclusive and royalty-free license to use such Intellectual Property as part of the Data Reports for Client's own internal business and operational purposes, including for the purposes of adapting, modifying, maintaining, updating, enhancing or further developing the Data Reports.
Data; Reports. The PSR Unit shall prepare monthly PSR data reports, by complaint category and vehicle mode, to provide statistics on system performance. Analysis of data by different record fields, eg. Vehicle mode, route/line, occurrence time, occurrence location, operators and equipment type, etc., shall be included as an addendum to the Monthly Report. Monthly PSR Data Analysis Reports shall be distributed to each Division Superintendent, SOM Service Operations, Schedules and Division Union Chair.
