Data Export upon Termination. Customer shall pay an export fee equal to 2 X’s the average monthly charges over the previous 90 days. Customer may elect any or a combination of the following data export options: CSV, CCDA, CDA, HL7, PDF, or PDF with a viewer.
Data Export upon Termination. Upon Expiry or termination we will provide the customer with 30 days’ access to the site to export their data in CSV or Excel format, and download all attachments stored in the system at no additional cost. Should the customer require access to the site past 30 days, additional fees will be prorated based on subscription fee.
Data Export upon Termination. Upon termination of this Agreement, and subject to the license in Section 2.2, Arthrex will export a file of each Participating AOSSM Member’s AOSSM Member Data to such Participating AOSSM Member in a mutually agreeable format and in a secure manner, and then erase all of such Participating AOSSM Member’s identifiable data from SOS; provided that, subject to applicable laws, Arthrex may retain a copy of such AOSSM Member Data for archival and dispute resolution purposes, as well as pursuant to the license granted in Section 2.2. Additionally, upon termination of this Agreement Arthrex will export the AOSSM Program Dataset to AOSSM in a mutually agreeable format and in a secure manner from SOS. The de- identified Participating AOSSM Member data will remain in the Combined Global Dataset.
