Customer Surveys. By providing Buyer’s email address(es) and/or phone number(s) on the signature page below, Buyer hereby agrees to participate in up to three (3) customer surveys (“Surveys”) from Seller during the one (1) year following Closing. Buyer authorizes Seller (including its parent entities, affiliates, and their service providers) to use automated or nonautomated means to contact Buyer at the email address(es) or phone number(s) provided for the purposes of conducting the Surveys and to use the contact information to improve its offerings.

Customer Surveys. The Contractor shall provide prompt, efficient, continuous and professional service to its Customers. Upon the request of the County, as part of the annual review of performance described above, the Contractor shall conduct a survey or surveys of Customers to determine their satisfaction with the Contractor’s service, including, without limitation, response to Customer complaints. The survey methodology, format, and content shall be subject to the prior review and approval of the County. A copy of the survey results shall be sent to the County within sixty (60) days of completion of the survey. Nothing in this paragraph shall limit the right of the County to conduct additional surveys at its own expense. The Contractor shall reasonably cooperate with the County in such cases. The County and Contractor shall review results of the survey and use these results to discuss improvements in service delivery. The cost of the Contractor’s survey will be treated as an allowable cost for rate setting purposes.
Customer Surveys. The second Transfer Benchmark shall be sustaining levels of support and client acceptance satisfactory to SBCL, in its reasonable discretion, determined by comparing Transfer Surveys of the Automated Providers in each Transferred Region to a corresponding Initial Survey for such Automated Providers. The Initial Survey shall be a survey, in a format and with content approved by SBCL in advance, which shall be performed by ActaMed within thirty (30) days after each Transfer Date. Such survey shall solicity performance and service-related comments from the Automated Providers about SBCL's provision of Lab EDI Services for that Region. The initial survey shall be sent to ten percent (10%) of the Sites in each Region, selected at random by ActaMed. The Transfer Surveys for each Region shall be identical to the Initial Survey for such Region and shall be sent to the same Automated Providers, to the greatest extent possible, as the Initial Survey (and to replacement Automated Providers where not possible). The Transfer Surveys shall solicit performance and service-related comments from the Automated Providers about ActaMed's provision of Lab EDI Services for each Region. The Transfer Surveys shall be performed for each applicable Region within thirty (30) days prior to the each scheduled Transfer Date; provided, that a Transfer Survey for a given Region shall only solicit information pertaining to the period between the last Transfer Date and the next scheduled Transfer Date. CUSTOMER COMPLAINTS The third and final Transfer Benchmark will be the absence of a material number (materiality to be determined by SBCL in its reasonable discretion relative to the frequency and severity of complaints) of documented problems and Automated Providers ceasing to do business with SBCL Labs citing issues related to Lab EDI Services provided by ActaMed. SBCL will provide ActaMed with copies of any such documented problems within fifteen (15) days of their receipt.
Customer Surveys. The parties will collaborate and cooperate with each other to reasonably coordinate any IESO customer surveys and LDC customer surveys.
Customer Surveys. All customer survey instruments and collection processes shall be approved by University’s Contract Administrator prior to survey administration. Such surveys may be conducted by Microsoft or a third party survey vendor selected by Microsoft ("Surveyor") on a quarterly basis, or more or less frequently as reasonably determined by Microsoft. Company shall cooperate with Surveyor in providing the names, e-mail addresses and phone numbers of all Customers who have contacted Company for support during the survey period, and this data shall be provided within twenty-four (24) hours from the time of the Customer contacting Company. The Surveyor, at Microsoft's expense, will select the respondents, call back after service has been delivered, administer the survey, calculate the results and report the results to Microsoft and Company within a mutually agreed upon time frame. In the event that Company's actual performance is less than the Quality Service Requirement, a deduction will be credited against the total amount of the per minute rate applicable for that particular month (or other time period as may be determined by Microsoft) as specified in Section 4(a). In the event that Company's actual performance is greater than the Quality Service Requirement, a positive adjustment will be made to the amount of the per minute rate applicable for that particular month (or other time period as may be determined by Microsoft) as specified in Section 4(a).
