COVID-19 Vaccination Sample Clauses

COVID-19 Vaccination. To the extent this Contract requires contractor, employees of contractor, or any subcontractor to perform any type of in- person work, including, but not limited to, the practice of medicine and health care, consulting, training, construction, maintenance, and vendor services, on property owned or leased in any capacity by the Board of Regents and/or CSN, or at any event sponsored by the Board of Regents or CSN, any and all employees of contractor or subcontractor who will perform such in-person work must have completed a COVID-19 vaccination series before they will be permitted on the aforementioned property. By executing this Contract, contractor represents and warrants that it is solely responsible for compliance with this provision and such compliance shall occur before any contractor or subcontractor employee covered by this provision enters Board of Regents or CSN property. Failure to comply with this section shall be deemed a material breach of this Contract, which entitles CSN to exercise its termination rights and relieves CSN of any payment obligation under the terms of this Contract. Failure to comply with this provision shall not be deemed a valid excuse or justification for contractor to fail to timely complete the work specified herein. A “completed COVID-19 vaccination series” is given the same meaning as in Title 2, Chapter 12, Section 12.4(c) of the Nevada System of Higher Education Code, a copy of which is attached hereto as Schedule 1. Within 72 hours of CSN’s written request to Contractor, Contractor shall provide to CSN, verification, acceptable to CSN in its sole discretion, of a completed COVID-19 vaccination series for any and all employees of contractor or subcontractor performing such in-person work. COVID-19 Vaccination. The Company requires COVID-19 vaccinations as a condition of employment. If you have any questions about COVID-19 vaccinations, you should consult a trusted healthcare provider. Unless an exemption applies, the Company requires that all employees be fully vaccinated before their Start Date. An employee is considered “fully vaccinated” if at least 14 days or a period specified by the vaccine’s manufacturer have elapsed since the employee received the last dose of a vaccine that the FDA has authorized for use in the United States. FDA-authorized vaccinations include vaccinations that have been authorized pursuant to an Emergency Use Authorization. For an employee fully vaccinated outside of the United States, the vaccination must be listed for emergency use by the World Health Organization (“WHO”). To establish that you are fully vaccinated, we require that you provide evidence of immunization by presenting a completed COVID-19 Vaccine Card or documentation from an authorized healthcare provider or pharmacy proving that you have received the COVID-19 vaccine. Please submit these documents to Xxxxxxx Xxxxxxxx at xxxxxxxxx@xxxxxxxxx.xxx or via the Company’s HR portal. The Company must receive this documentation prior to your projected Start Date. Employees who have i) a qualifying medical condition that contraindicates the vaccination, or ii) who object to being vaccinated on the basis of a sincerely held religious belief or practice, may contact Xxxxxxxx Xxxx at xxxxx@xxxxxxxxx.xxx. The Company regards all such information as confidential.

The Board of Trustees has mandated that all faculty members who work on site must be fully vaccinated against COVID-19, unless an exception is granted for a medical or religious reason. Faculty members who have a medical or religious reason must notify the District to provide documentation supporting their status.

Vaccination includes ato complete their primary series of COVID-19 vaccines (i.e. be fully vaccinated, does not include boosters) as recommended , additional doses, and boosters that are recommended by the U.S. Centers for Disease Control and Prevention, or be approved for medical or religious exemption and accommodation as a condition of employment. . Employees who fail to maintain this condition of employment for their position will be subject to non-disciplinary separation.

COVID-19 Vaccination. This position requires work onsite at the Company’s facilities and/or in-person interaction with the Company’s employees and/or partners. In furtherance of its duty to its employees to provide and maintain a workplace that is free of known hazards, the Company requires that all employees who work in its facilities or who interact in-person with its employees and/or customers to be fully vaccinated against COVID-19, subject to reasonable accommodations for qualifying medical or religious reasons, and/or as otherwise required by applicable law. Accordingly, this offer and your employment with the Company is contingent upon satisfactory proof that you are fully vaccinated from COVID-19 as of your employment start date, unless a reasonable accommodation is approved.

Boosters are not mandatory, however, employees who provide proof of up-to-date COVID- 19 vaccination, to include boosters, may receive a one-time lump sum payment, pursuant to Article 32.30.
