Cost of Life Insurance Coverage. Employees eligible for the life insurance coverage described under Section 1 of this paragraph, above, who elect such coverage, shall pay to the City an amount equal to $0.21 per month for each $1,000 of coverage in excess of $ 25,000. These payments shall be accomplished by periodic deductions from employees' biweekly paychecks. The City shall make all other necessary payments for the life insurance coverage described in Section 1, of this paragraph, above.

Cost of Life Insurance Coverage. Employees eligible for the basic life insurance coverage described under Subsection 1of this Article, above, who elect additional life insurance coverage, shall pay to the City an amount equal to a cost per thousand each month, at age banded rates, for each $1,000 of coverage in excess of $55,000. These payments shall be accomplished by periodic deductions from employees' biweekly paychecks. The City shall make all necessary payments for the life insurance coverage described in Section 1. of this Article, above. Employees who are enrolled in additional life insurance coverage may also purchase life insurance for spouses and dependents, at no cost to the City.
Cost of Life Insurance Coverage. Eligible employees who elect life insurance coverage, shall pay the following amount to the City: an amount equal to $0.21 per month for each $1,000 of coverage in excess of $30,000 but not greater than 1.5 times his/her annual basic salary rounded to the next higher thousand dollars of earnings and an amount equal to the full premium per month for each $1,000 of coverage in excess of 1.5 times his/her annual basic salary rounded to the next higher thousand dollars. These payments shall be accomplished by periodic deductions from employees' biweekly pay checks. The City shall make all other necessary payments for the life insurance coverage described under subsection 1, above.
Cost of Life Insurance Coverage. An employee eligible for the life insurance coverage described under 38.1., above, who elects such coverage, shall pay to the City an amount equal to $0.21 per month for each $1,000 of coverage in excess of $35,000, but not greater than 1.5 times his/her annual base salary rounded to the next higher thousand dollars and an amount equal to the full premium per month for each $1,000 of coverage in excess of 1.5 times his/her annual base salary rounded to the next higher thousand dollars. These payments shall be accomplished by periodic deductions from employees' biweekly paychecks. The City shall make all other necessary payments for the life insurance coverage described under 38.1., above.

  • Insurance Cover Without prejudice to the provisions contained in Clause 26.1, the Concessionaire shall, during the Operation Period, procure and maintain Insurance Cover including but not limited to the following:

  • Life Coverage Paragraph 1: The Board shall provide a group term life coverage in the sum of $30,000 for all teachers employed half time or more. Any increases in coverage shall not be effective until the teacher reports or is able to report for work.

  • Single Coverage The School District will pay up to $28.00 per month for individual coverage for each full-time teacher who qualifies for and enrolls in the School District's group dental insurance plan.

  • Required Insurance Coverage As a condition of this Contract with DIR, Vendor shall provide the listed insurance coverage within 5 business days of execution of the Contract if the Vendor is awarded services which require that Vendor’s employees perform work at any Customer premises and/or use employer vehicles to conduct work on behalf of Customers. In addition, when engaged by a Customer to provide services on Customer premises, the Vendor shall, at its own expense, secure and maintain the insurance coverage specified herein, and shall provide proof of such insurance coverage to the related Customer within five (5) business days following the execution of the Purchase Order. Vendor may not begin performance under the Contract and/or a Purchase Order until such proof of insurance coverage is provided to, and approved by, DIR and the Customer.   • Maintenance of Insurance Coverage Each party agrees to maintain throughout the term of this Agreement professional liability insurance coverage of the type and amount reasonably customary in its industry. Upon request, a party shall furnish the other party with pertinent information concerning the professional liability insurance coverage that it maintains. Such information shall include the identity of the insurance carrier(s), coverage levels, and deductible amounts.

  • Health Insurance Coverage (a) An employee who is laid off or separated from employment on or after July 1, 1994, under circumstances which entitle such employee to reemployment rights under this Article, other than pursuant to Section 23, may elect to continue membership in their health benefit plan, upon advance payment of the regular percentage contribution to the cost of the plan, during the first six
