Cosmetic Procedures. Custodial or domiciliary care—includ- ing services and supplies that assist members in the activities of daily living such as walk- ing, getting in and out of bed, bathing, dress- ing, feeding, and using the toilet; preparation of special diets; supervision of medication that is usually self-administered; or other ser- vices that can be performed by persons who are not providers.

Cosmetic Procedures. Payment is due and payable at the time of service.
Cosmetic Procedures for any operations or procedures done for cosmetic purposes to improve the appearance of any portion of the body, and from which no improvement in physiological function can be expected, except as otherwise required by law. Other exceptions to this exclusion are: a) surgery to correct a condition resulting from an accident; and b) surgery to correct functional impairment which results from a covered disease, injury, or congenital birth defect.
