COSHH. To comply with the Control of Substances Hazardous to Health 1999, Contractors must provide written risk assessments and safety data sheets that must be available for his employees to see, of all substances used in his undertaking, which may effect the health of his employers, prison staff and visitors of the establishment and a copy of the assessments must be submitted to the Health and Safety Advisor (if appointed) of the establishment. Otherwise to the Establishments Site Manager.

COSHH. The Supplier shall in relation to all chemicals owned and provided by it: • Supply to and display in the Site a full and regularly updated COSHH policy indicating the chemicals regulated from time to time by COSHH which are used or intended to be used at the Site. • Retain MSDS’s and risk assessments for all chemicals used, in a register on Site. • Supply and display, adjacent to the place where chemicals regulated from time to time by COSHH are stored, all wipe clean wall chart summarising important COSHH information in order that the necessary first aid measures can be taken in the event of the accident involving such chemicals. • Keep all chemicals regulated by COSHH in secure containers fully and accurately labelled in accordance with all applicable labelling regulations and any other relevant regulations in force from time to time. • Ensure that only safe systems of work, supported by appropriate risk assessments are adopted by the Staff and that the Staff are all fully trained in the requirements of COSHH. • Ensure that DCC’s employees, suppliers, customers and all third parties are protected from any hazardous operation carried out by the Supplier.

  • Orthodontics We Cover orthodontics used to help restore oral structures to health and function and to treat serious medical conditions such as: cleft palate and cleft lip; maxillary/mandibular micrognathia (underdeveloped upper or lower jaw); extreme mandibular prognathism; severe asymmetry (craniofacial anomalies); ankylosis of the temporomandibular joint; and other significant skeletal dysplasias. Procedures include but are not limited to: • Rapid Palatal Expansion (RPE); • Placement of component parts (e.g. brackets, bands); • Interceptive orthodontic treatment; • Comprehensive orthodontic treatment (during which orthodontic appliances are placed for active treatment and periodically adjusted); • Removable appliance therapy; and • Orthodontic retention (removal of appliances, construction and placement of retainers).

  • Wellness A. To support the statewide goal for a healthy and productive workforce, employees are encouraged to participate in a Well-Being Assessment survey. Employees will be granted work time and may use a state computer to complete the survey.

  • Footwear Where an employee is required by the College or by legislation, in order to perform his/her duties, to acquire and wear protective footwear, the employee shall provide the College with proof of purchase by March 1 each year and the College shall reimburse such employee, on the first pay day in April in each year, up to a maximum of one hundred and fifty dollar ($150.00). In situations other than the foregoing, the College may, in its discretion, (which discretion shall not be unreasonably exercised) reimburse such expense where it is recommended by the health and safety committee constituted under the Occupational Health and Safety Act.

  • Prosthodontics We Cover prosthodontic services as follows: • Removable complete or partial dentures, for Members 15 years of age and above, including six (6) months follow-up care; • Additional services including insertion of identification slips, repairs, relines and rebases and treatment of cleft palate; and • Interim prosthesis for Members five (5) to 15 years of age. We do not Cover implants or implant related services. Fixed bridges are not Covered unless they are required: • For replacement of a single upper anterior (central/lateral incisor or cuspid) in a patient with an otherwise full complement of natural, functional and/or restored teeth; • For cleft palate stabilization; or • Due to the presence of any neurologic or physiologic condition that would preclude the placement of a removable prosthesis, as demonstrated by medical documentation.
