Community Mental Health. Agency agrees to do the following: • Screen any child for CMH service eligibility via an appointment or walk-in. • Conduct a routine screenings within 14 calendar days, with the first service delivered within 14 days of screening. • Accommodate urgent situations at the Center, or in the Emergency Room by CMH Mobile Crisis either during or after business hours. Emergent situations (e.g., active suicidal plan) should be referred to the Emergency Room, not CMH. • Accept referrals from school into services when appropriate and consistent with CMH eligibility criteria, and pending the availability of services when Medicaid is not the payer, utilizing referral process as agreed upon. • Assure that eligibility for specialty services and supports shall not be limited on the basis of race, color, religion, national origin, age, gender, sexual orientation, political affiliation, income level, insurance status, ability to pay, or disability. • Maintain the confidentiality of the information received in accordance with the signed release of information. • Communicate with designated members of the referred child’s team as authorized through a signed release of information. • Provide education to school staff as requested through CMH Customer Services. • Provide educational groups for parents within the school setting as requested through CMH Customer Services. • Designate a staff liaison in the Center to work with school on student referrals. • Share information regarding changes in CMH policy or procedure that may affect referral processes or services with as much advance notice as possible. • Participate in routine Quality Improvement practices according to established QI policies. Outcomes pertinent to this Collaboration will be communicated with School as necessary. • The Chief Operating Officer of CMH will meet with School liaisons to review this agreement on a regular basis (i.e. quarterly) to evaluate effectiveness of components of this Agreement. Any adjustments identified, agreed upon by both parties, will be made and documented.

Community Mental Health. Integrated Community Mental Health Services provide specialist crisis, maintenance and counselling services to mental health consumers in the community. This service is provided for consumers aged 16-64 years. The Inner South Community Mental Health Centre is at Xxxxxx, and the Outer South Community Mental Health Centre is at Noarlunga. Older persons’ community mental health provides service to consumers aged 65+ years and is located at Edwardstown. Veterans’ mental health community services located at Glenside Health Service Campus include Post-Traumatic Stress Disorder services, hospital outpatient programs and consultation liaison services. Other Community Provision of counselling services to mental health consumers is provided utilising the Clinical Psychology Service at FMC. Improving Access to Psychological Therapy: Short-term brief intervention services based within FMC Emergency Department for the provision of support for consumers who experience Anxiety and Depression. Centre for Anxiety Related Disorders: Specialist Cognitive Behaviour Therapy service provision based in FMC.
Community Mental Health. Provides services to adults who experience acute mental illness, adults with severe and persistent mental illness, and older adults with mental health problems. Employment-related services are delivered by Community Mental Health Workers, Intensive Case Managers, Employment Development Counsellors, Proctors, and Program of Assertive Community Treatment (PACT) multidisciplinary outreach teams. In 2017/18, the Community Mental Health Program provided employment-related services to 1,056 individuals.

  • Community Service Service to the wider community includes active participation in a wide variety of governmental, societal and community institutions, programs and services, where such participation is based on the candidate’s academic or professional expertise.

  • Mental Health The parties recognize the importance of supporting and promoting a psychologically healthy workplace and as such will adhere to all applicable statutes, policy, guidelines and regulations pertaining to the promotion of mental health.

  • Home Health Care This plan covers the following home care services when provided by a certified home healthcare agency: • nursing services; • services of a home health aide; • visits from a social worker; • medical supplies; and • physical, occupational and speech therapy.

  • Mental Health Services This agreement covers medically necessary services for the treatment of mental health disorders in a general or specialty hospital or outpatient facilities that are: • reviewed and approved by us; and • licensed under the laws of the State of Rhode Island or by the state in which the facility is located as a general or specialty hospital or outpatient facility. We review network and non-network programs, hospitals and inpatient facilities, and the specific services provided to decide whether a preauthorization, hospital or inpatient facility, or specific services rendered meets our program requirements, content and criteria. If our program content and criteria are not met, the services are not covered under this agreement. Our program content and criteria are defined below.

  • Community Engagement The HSP will engage the community of diverse persons and entities in the area where it provides health services when setting priorities for the delivery of health services and when developing plans for submission to the LHIN including but not limited to CAPS and integration proposals. As part of its community engagement activities, the HSPs will have in place and utilize effective mechanisms for engaging families, caregivers, clients, residents, patients and other individuals who use the services of the HSP, to help inform the HSP plans, including the HSP’s contribution to the establishment and implementation by the LHIN of geographic sub-regions in its local health system.

  • Normal Commercial Relations Anything contained in this Trust Indenture to the contrary notwithstanding, the Owner Trustee, the Indenture Trustee, any Participant or any bank or other Affiliate of such Participant may conduct any banking or other financial transactions, and have banking or other commercial relationships, with Lessee, fully to the same extent as if this Trust Indenture were not in effect, including without limitation the making of loans or other extensions of credit to Lessee for any purpose whatsoever, whether related to any of the transactions contemplated hereby or otherwise.

  • Extended Health Care Plan (a) The Employer shall pay the monthly premium for regular employees entitled to coverage under a mutually acceptable Extended Health Care Plan.

  • Behavioral Health Services Behavioral health services include the evaluation, management, and treatment for a mental health or substance use disorder condition. For the purpose of this plan, substance use disorder does not include addiction to or abuse of tobacco and/or caffeine. Mental health or substance use disorders are those that are listed in the most updated volume of either: • the Diagnostic and Statistical Manual of Mental Disorders (DSM) published by the American Psychiatric Association; or • the International Classification of Disease Manual (ICD) published by the World Health Organization. This plan provides parity in benefits for behavioral healthcare services. Please see Section 10 for additional information regarding behavioral healthcare parity. Inpatient This plan covers behavioral health services if you are inpatient at a general or specialty hospital. See Inpatient Services in Section 3 for additional information. Residential Treatment Facility This plan covers services at behavioral health residential treatment facilities, which provide: • clinical treatment; • medication evaluation management; and • 24-hour on site availability of health professional staff, as required by licensing regulations. Intermediate Care Services This plan covers intermediate care services, which are facility-based programs that are: • more intensive than traditional outpatient services; • less intensive than 24-hour inpatient hospital or residential treatment facility services; and • used as a step down from a higher level of care; or • used a step-up from standard care level of care. Intermediate care services include the following: • Partial Hospital Program (PHP) – PHPs are structured and medically supervised day, evening, or nighttime treatment programs providing individualized treatment plans. A PHP typically runs for five hours a day, five days per week. • Intensive Outpatient Program (IOP) – An IOP provides substantial clinical support for patients who are either in transition from a higher level of care or at risk for admission to a higher level of care. An IOP typically runs for three hours per day, three days per week.

  • Residence Community Living Standards The Residence Community Living Standards (“RCLS”) forms a part of this Agreement. It details the rights, responsibilities, and privileges of Residents as well as the residence conduct process. Each Resident is responsible for reading, understanding, and adhering to the terms outlined within the RCLS. The Manager and the Institution may amend the terms of the RCLS from time to time and may post the amendments in the Residence. Failure to abide by the RCLS may result in eviction from Residence and termination of this Agreement as per the violations and sanctions outlined in the RCLS, and as stated in section 8.01(d).
