Chronic Conditions. Employees who suffer from chronic or recurring illnesses or disabling conditions that do not require a visit to a health care provider each time the condition is manifested, shall not be required to provide certification for each absence, provided that a general certification is provided unless the absence is for five (5) or more consecutive days. Such frequent absences shall also not be used as the basis for a certification requirement. Unless the employee has a condition identified as a permanent disabling condition, the Employer may require certification and follow-up reports from a health care provider no more frequently than every six (6) months of the continued existence of the chronic condition.

Chronic Conditions. If the insured person is insured under the Essential policy option, the insured person does not have cover for costs relating to the maintenance of chronic conditions. For Advance, Excel and Apex policy options, cover up to the limits in the benefit schedule are a maximum limit per period of cover and not per medical condition.

Chronic Conditions. Employees who are suffering from a chronic condition which may require the use of unscheduled sick leave on a regular and recurring basis, must establish the chronic nature of their illness with medical documentation. The documentation shall indicate how often it is anticipated that the employee may be incapacitated by the chronic condition and the likelihood that the condition may or may not continue and/or its expected duration. In those instances, the employee will not normally be required to provide medical documentation for additional sick leave usage, related to the chronic condition.
Chronic Conditions. Managers will use discretion before imposing a Letter of Requirement on an Employee who suffers from a medical condition documented in a medical certificate or physician’s diagnosis as a chronic or long-term condition during the duration of that condition or when the duration of the condition is uncertain.

Chronic Conditions. An employee shall be allowed to use sick leave for time spent on Family Medical Leave (FMLA). The employee and the Employer shall comply with the dictates of the FMLA when such leave is requested. Employees and/or dependent as defined in Section 2.1.(b)who suffer from chronic or recurring illnesses or disabling conditions that constitute Family Medical Leave Act shall not be required to provide documentation for each absence, provided that a general certification has been previously provided. Such absences shall not be used as the basis for a documentation requirement described in Section 4.2. Unless the employee (and/or dependent) has a condition identified as a permanent disabling condition, the Employer may require documentation from a health care provider in accordance with the FMLA.
