Certificates of Insurance and Additional Insured Endorsements. Prior to the commencement of any Work under this contract, Contractor and its Subcontractors of any tier shall provide to the School District a Certificate of Insurance on Xxxxx Form 25 outlining and showing that it is in compliance with all of the insurance provisions outlined in this section including both coverage and limits. The policy shall contain a provision that coverage afforded thereunder shall not be canceled or non- renewed or restrictive modifications added, without forty- five days prior written notice to the School District, except for any applicable state mandated provision for non-payment of premiums. Contractor and its Subcontractors of any tier working on Contractor’s behalf or request, shall provide and name the School District and its Project Manager and A/E as additional insureds as per form CG 2010 1185, or both a CG 2010 1001 and CG 2037 1001 or equivalent. Additional insured coverage shall apply to: (i) Contractor’s Commercial General Liability Insurance, (ii) its Commercial Auto Liability Insurance, (iii) any policy(s) providing Excess or Umbrella Liability coverage. The insurance afforded to the additional insureds shall be primary insurance and not excess over, or contributing with, any insurance purchased or maintained by additional insureds. Such additional insured coverage shall also confer status or contain a waiver of subrogation in favor of any and all additional insureds. All additional insured endorsements and coverage shall apply to both ongoing and completed operations. Contractor is required to continue naming the School District and its Project Manager and A/E as an additional insured with all the provisions outlined above, through the applicable Statute of Repose or Statute of Limitations; whichever is greater. The School District’s acceptance of any Certificate of Insurance shall not relieve, nor constitute a waiver of Contractor’s obligations hereunder.

Certificates of Insurance and Additional Insured Endorsements. Independent Contractor shall provide to Company certificates of insurance for Independent Contractor’s general liability, automobile liability, worker’s compensation and collision coverage. Independent Contractors shall procure general liability and automobile liability coverage with limits not less than those enumerated in Addendum A, and such policy(ies) shall identify Company as an additional insured.
Certificates of Insurance and Additional Insured Endorsements. The Parties shall provide each other with certificates of insurance, and if requested, certified copies of the policies required by paragraphs B, C and D of this Article. Each Party shall also provide Additional Insured Endorsements for the insurance required hereby on forms ISO CG 2010 or ISO 2033 or their equivalents. The certificates shall provide that 30 days written notice of any reduction in coverage or cancellation of the insurance will be provided, and the certificates of insurance required and the underlying policies therefore, shall expressly include the other Party to be named as an additional insured thereunder. Certificates of Insurance and Additional Insured Endorsements. (a) Within fifteen (15) days of the execution of this Agreement and at time of renewal, Licensee shall furnish (i) a Certificate of Insurance evidencing all coverages described herein, and (ii) additional insured endorsements attached thereto, listing LS&Co. The policy provision(s) or endorsement(s) must further provide that coverage is primary and not excess over or contributory with any other valid, applicable, and collectible insurance or self-insurance in force for the additional insureds.

  • Additional Insured City, its officers, agents, employees, and volunteers are to be included as insureds using the applicable ISO additional insured endorsement(s) or substitute forms providing equivalent coverage, in respects to damages and defense arising from: activities performed by or on behalf of Contractor, including the insured's general supervision of Contractor; products and completed operations of Contractor; premises owned, occupied, or used by Contractor. The coverage shall contain no special limitations on the scope of protection afforded to City, its officers, employees, or volunteers. Additional insured status for City shall apply until the expiration of time within which a claimant can bring suit per applicable state law. Any failure by the Contractor to comply with reporting provisions of the policies shall not affect its obligations to the additional insureds.

Certificates of Insurance shall:  Be in the form acceptable to OGS and in accordance with the New York State Insurance Law (e.g., an XXXXX certificate);  Disclose any deductible, self-insured retention, aggregate limit or exclusion to the policy that materially changes the coverage required by this Contract;  Refer to this Contract by award number;  Be signed by an authorized representative of the referenced insurance carriers; and  Contain the following language in the Description of Operations / Locations / Vehicles section: The People of the State of New York, the New York State Office of General Services, any entity authorized by law or regulation to use the Contract and their officers, agents, and employees are included as an additional insured on endorsement CG 20 10 11 85 (or endorsements that provide equivalent coverage, such as the combination of CG 20 10 04 13 (covering ongoing operations) and CG 20 37 04 13 (covering completed operations)), and General liability coverage is provided on the current edition of Commercial General Liability Coverage Form CG 00 01 (or a form that provides equivalent coverage). Additional insured protection afforded is on a primary and non-contributory basis. A waiver of subrogation is granted in favor of the additional insureds. Only original documents (certificates of insurance and any endorsements and other attachments) or electronic versions of the same that can be directly traced back to the insurer, agent or broker via e-mail distribution or similar means will be accepted. OGS generally requires Contractors to submit only certificates of insurance and additional insured endorsements, although OGS reserves the right to request other proof of insurance. Contractors are requested to refrain from submitting entire insurance policies, unless specifically requested by OGS. If an entire insurance policy is submitted but not requested, OGS shall not be obligated to review and shall not be chargeable with knowledge of its contents. In addition, submission of an entire insurance policy not requested by OGS does not constitute proof of compliance with the insurance requirements and does not discharge Contractors from submitting the requested insurance documentation.
