CAMP FIELD TRIP RELEASE. I hereby give the staff of Camp WOOF! Daycare my express permission to take my above-states pet/s on trips outside of the facility. This includes but is not limited to car rides, walks and trips to the park. By agreeing to the above I am stating my above pet is safe to be around other vehicles, strangers, and animals. I assume full responsibility for any injuries or damage caused by my animal due to undisclosed temperament or reactions, which can include but are not limited to people, property, and other animals.
