BENEFITS AND SERVICES. Purpose Of This Provision A Member may be covered under this Contract and subsequently become covered by or eligible for coverage under Medicare or coverage under a group Health Benefits Plan, Group Health Plan, Governmental Plan, or Church Plan. This provision allows Us to coordinate the services and supplies We provide with what Medicare pays or what Medicare would pay. This provision also allows us to coordinate benefits with what a group Health Benefits Plan, Group Health Plan, Governmental Plan, or Church Plan pays. Coordination of benefits is intended to avoid duplication of benefits while at the same time preserving certain rights to coverage under all Plans under which the Member is covered. Please note: The ONLY circumstances in which a person may be covered under both this Contract and under Medicare or coverage under a group Health Benefits Plan, Group Health Plan, Governmental Plan, or Church Plan occur when a Member is already covered under this Contract and subsequently becomes eligible for Medicare or coverage under a group Health Benefits Plan, Group Health Plan, Governmental Plan, or Church Plan.

For instance, he or she may be covered by this [Contract] as an Employee and by another plan as a Dependent of his or her spouse. If he or she is covered by more than one Plan, this provision allows Us to coordinate what We pay or provides with what another Plan pays or provides. This provision sets forth the rules for determining which is the Primary Plan and which is the Secondary Plan. BENEFITS AND SERVICES. Your membership fee entitles you to the following benefits that the Provider agrees to provide, specified under “Membership benefits.” This membership, including all consultations relevant to the membership option chosen, is exclusive to you and may not be transferred to your spouse, partner or other dependants. It remains your responsibility to submit the bill for all consumable products used to your Medical Aid/Health Insurer for re-imbursement, should you so desire. Your medical coverage, deductibles and co-payments will apply in typical fashion. Your enrollment in the membership will continue until this Membership Agreement is terminated.

BENEFITS AND SERVICES. (1) An eligi- ble veteran on whose behalf payments are made to the employer shall be pro- vided all other Chapter 31 benefits and services furnished to other veterans re- ceiving employment services. All of Team1Medical’s clients, participate in our Quality Assurance Program. A portion of this program is designed to measure the soft skills and performance of our contingent labor through their assignment. Customized to fit your requirements, Team1Medical’s Quality Control may include: • Arrival confirmation • 1st day quality check call • 1st week evaluation • Periodic scheduled and unscheduled calls • Participation in Team1Medical’s PERC program (Program for Employee Relations and Customer Service) • Quality Reviews with Team1Medical’s Management team If during the performance monitoring, we discover areas in which our employees require improvement in order to be totally successful in their position, a Team1Medical staffing specialist will counsel the employee off-site through proven professional methods. However, in the event a termination does occur, Team1Medical will contact the employee after work hours to end the assignment. Simply notifying a Team1Medical representative by phone will ensure receipt of the information for immediate action. The College practices on pensions, sick pay, etc should be monitored to ensure there is no direct or indirect discrimination against women. Access to overtime should be checked to ensure that women and men have equal access to this work, and an equal right to refuse non-contractual overtime Complaints The College should seek to provide a supportive environment for staff who wish to make a claim of pay inequality. Review and Consultation This Joint Agreement on Guidance should be reviewed on a regular basis in accordance with legislative developments and the need for good practice, by the College. As part of the review the College should seek and take into account the views of stakeholders including the consultation/negotiating arrangements within the College, and appropriate equality bodies.
