Becoming a Donor. An employee who voluntarily wishes to donate accumulated PTO hours completes a Leave Sharing Plan Donor form which is available in the Human Resources Department. The form is forwarded to Human Resources for processing with the Payroll Department. The Payroll Department will affect the transfer of the balances between employees.
