Assessment Form. 1. The assessment form contained in Appendix I is used to document performance ratings. It is used for all instructional personnel. The form is completed by the assessor and signed by the employee. The employee’s signature indicates only that he/she has read the form and does not necessarily indicate agreement with its content.
