Applications for Leave. An employee or his or her authorized representative may request Organ Donation Leave by submitting: (1) the appropriate request form and (2) the medical documentation required as indicated on the form, to the Executive Director of Human Resources. The College shall render its decision on the request for Organ Donation Leave within five (5) business days of the Executive Director of Human Resources’ receipt of the request. Leave shall be provided retroactively if the employee had to make his or her donation before receiving approval and the leave is otherwise approved in accordance with this Article. If there is any question about medical documentation, it shall be forwarded to the State Medical Review Director for review. Organ Donation Leave shall be denied only if the employee fails to provide the medical documentation that is required to establish his or her status as a donor.

Applications for Leave. All applications for union leave of absence whether with or without pay shall be granted to Official Union Representatives provided their absence does not interfere with the Employer’s ability to meet operational needs. A minimum of twenty-four (24) hours’ notice shall be provided when requesting a leave of absence. Such notice shall be waived to accommodate the three (3) hour notice period contained in Clause 14 when a meeting is called by the Employer for the purposes of written discipline, suspension or dismissal and may be modified or waived at the discretion of the Employer in other situations.
Applications for Leave. Applications for sabbatical leave must be filed with the Superintendent no later than four (4) calendar months prior to the beginning of the semester during which the leave would commence.
Applications for Leave. A request for Family Medical Leave must be made in writing by completing the Family Medical Leave application form. The application must be submitted to the employee's administrator and then forwarded to the Office of Human Resources at least thirty days before the requested start of the leave unless the reason for the leave is due to an emergency, in which case the request must be made immediately. For leaves of absence of twenty (20) or more working days for Regular Full-Time Employees and of four (4) consecutive weeks or more for Regular Part-Time Employees (“Long Leaves”), the application shall be submitted to the City Manager. In all cases each request will be judged based on the circumstances and merits of each application. The final and binding decision to grant Short General Leaves shall rest with the non-bargaining unit supervisor and the final and binding decision for Long General Leave shall rest with the City Manager. Under no circumstances will a leave be granted for an employee to engage in other employment or start a business. In the event an employee on leave under this Article takes a new job or starts a business, their employment with the City of Port Xxxxx will be terminated for cause. However, an employee who has a second job, or operates their own business prior to taking the leave, and does not work additional hours in the second job or in the business as a result of taking the leave, will not be terminated for cause for the sole reason that the employee continues to work at the other job or continues to operate their own business.

A false statement in an application for leave of absence, or neglect to return at the end of the leave may result in dismissal, which shall be reported to the Union. Leave of absence without pay, to a maximum of forty-five (45) days may be granted in case of serious illness or accident to the Regular employee’s immediate family, or for any other reason which the Employer and Regular employee agree upon, including extended vacations, marriage, education and professional or educational meetings. Permission for leave of absence will not be unfairly withheld, and where permission is denied reasons will be given. Such leave may be extended by the Employer for extenuating circumstances.
