Application to Participate. Each Employee who becomes eligible to participate in this Plan shall complete the forms and provide the data as required by the Employer as a condition to participate. The forms and data shall be delivered to the Employer prior to the first day of an Employee’s participation in the Plan, or at such earlier date (but not to exceed 30 days) prior to the first day of an Employee’s participation in the Plan, as required by the insurance company providing the applicable benefit described in Section 6.01.
Application to Participate. 1. All report cards, transcripts or other certification of completion of approved courses must be submitted to the Human Resources Division by October 15th of each school year for the professional growth stipend to be granted the following December.

Application to Participate. Prior to commencing professional growth activities, the employee shall submit an application to the Professional Growth Committee and receive approval. The application must include information why each course or activity relates to the employee’s current position or career goal. Employees preparing for an unpaid leave of absence for educational purposes are required to follow this procedure prior to taking the leave. See Appendix K for application.

An Eligible Employee shall also be required to execute a Salary Redirection Agreement, to elect to reduce salary to pay for allowable Benefits, during the Election Period for the Plan Year during which he wishes to participate in this Plan. Any such Salary Redirection Agreement shall be effective for the first pay period beginning on or after the Employee’s effective date of participation pursuant to Section 2.2. A failure to execute a Salary Redirection Agreement shall constitute an election by the Eligible Employee to receive his or her full salary or other compensation in lieu of Benefits available hereunder.
