Application for Leave. A. Eligible employees, who must meet the criteria in #4 above, may apply to use the leave bank by completing an application and a release for medical information form from their supervisor. In the event an employee is physically or mentally unable to initiate a request, the request may be initiated by a member of the employee's family, or by their department.
