Application for Leave Without Pay. Written applications are presented to the appropriate xxxx after consultation with the appropriate Chair or xxxx or management supervisor. For corrections academic employees, the application is presented to the Corrections Education Director. Applications are presented as early as possible, but not less than sixty (60) working days prior to the effective date of the intended leave. Applicants will receive notification of the President's decision within 15 working days. When necessary, management will consider special applications on a shorter term than indicated above.

Application for Leave Without Pay. Any request for leave of absence shall be submitted in writing by the employee to his immediate supervisor at least ten (10) working days prior to the date of departure if at all possible. The request shall state the reason the leave of absence is being requested and the length of time off that the employee desires. Authorization for a leave of absence must be in writing and must contain the signature of the employee's immediate supervisor. Any request for a leave of absence shall be answered promptly. Requests for immediate leaves (for example, immediate family sickness or death) shall be answered before the end of the shift on which the request is submitted. A request for a short leave of absence (defined as a leave not exceeding one month) shall be answered within five (5) working days. A request for a leave of absence exceeding one month shall be answered within ten (10) working days. No seniority, vacation benefits, sick-leave benefits or holiday benefits shall accrue during a leave of absence. Employees shall be returned to the position they held at the time the leave of absence was requested. Except as provided to the contrary in Section 11.03 of this Article, any employee on leave of absence shall not receive compensation from the Employer during such period. Leaves of absence not to exceed six (6) months may be granted by the Employer for any reasonable purpose and may be extended to a maximum of one (1) year. What constitutes a reasonable purpose in each case shall be determined by the Employer. Employees elected to any Union office or selected by the Union to do work which takes them from their employment with the Employer, shall, at the written request of the Union, be granted a leave of absence. The leave of absence shall not exceed one (1) year.
Application for Leave Without Pay. (a) Application for leave of absence without pay for any of the reasons cited in this provision, shall be filed by the employee, on the prescribed form, with the head of his/her department. Such application shall state the reasons for the requested leave and the duration thereof. If approved by the Head of the department, the application shall be submitted to the Commissioner of Personnel, and leave of absence shall be granted only when finally approved by the Commissioner of Personnel. It is understood that such employee will be permitted to return to the same class title with in the same department.
