Application and Fee Clause Samples

Application and Fee. Resident shall pay an Application Fee as described more fully at Exhibit A.

Application and Fee. Completion of a written membership enrollment application (the “Application”) and this Agreement in such form as may be prescribed by the Company, along with the initial fee and monthly membership fee. By completion of the Application, each prospective Member agrees to be bound by the terms of this Agreement.

Application and Fee. Resident shall pay an Application Fee and, depending on the type of Plan chosen by Resident, an Admission or Community Fee as described more fully at Exhibit A.
