APPENDIX 1A. Within 45 days after submittal of this petition, the Parties shall undertake to complete a form of document known as “living license,” in consultation with the SWRCB’s Office of Chief Counsel. This form will: (i) include all existing terms of the Licenses, (ii) show the Appendix 1 conditions as redline of those terms proposed to be amended, and (iii) also show the Appendix 1 findings and conclusions. Upon completion of this form, the Parties shall submit it as Appendix 1A, as a supplement to Appendix 1, for the SWRCB’s approval of the terms proposed to be amended.

APPENDIX 1A TO ANNEX A TO Contract No. 301726016 MEDICAL CLEARANCE PACKAGE INTRODUCTION: Vojenský výzkumný ústav, s.p. is committed to providing a safe place of work, while at the same time ensuring that training with live agents has value and is effective. Our approach to safety complies with the national legislation1) and international standard2) for safety management systems, and is based on an absolute aim to prevent any accidents in the workplace. In this respect, we ensure through the multiple layer of control that exercises and tasks conducted during Live Agent Training meet strict safety requirements. As Live Agent Training can be provided to psychically and mentally sound personnel only, all personnel who are supposed to undergo such training shall be fully medically examined. Reason for such examination is to exclude any personnel’s disorder, complaint or any other medical problems well in advance which could prevent them to stay in contaminated environment and/or to use of individual protective equipment (IPE). Although overall responsibility for the safe conduct of Live Agent Training lies with the facility provider, client does bear the part of responsibility for occupational safety and health protection measures which shall be taken in favour of his personnel before Live Agent Training commencement.
