Annual Survey. 4.2.1. After annual survey, the following information shall be kept on file in a format which can be accessed and downloaded electronically.  Confirmation that the classification certificate is retained,  Confirmation that survey, inspections and endorsements have been carried out as delegated in Annex VI to the Agreement. The confirmation shall include information about level and amount of inspections and surveys that have been carried out.

Annual Survey. The principal is to conduct an annual survey of the staff to determine the preference, in order of priority, of each person’s grade level and/or department assignment. This survey is to be completed by March 15 each year. The teacher is to indicate semester units and length of service in each of the designated areas of teaching preference. All preferences will be taken into consideration when filling vacant positions should a domino effect in positions occur.
Annual Survey. On-Site Rx shall conduct a survey(s) of non- identifiable, random customers to ascertain the level of customer satisfaction with the onsite pharmacy service and report the results to the City annually. The survey shall contain at least five questions assessing professionalism, accuracy of prescription, cleanliness and appearance of the facility, and availability of the prescribed medication shall be conducted according to the 5-point Likert Methodology (Poor, Below Average, Average, Above Average, and Excellent). On-Site Rx represents that it will achieve a satisfaction rate of Above Average or Excellent on eighty (80) percent of survey responses, and the failure on the part of On-Site Rx to achieve this satisfaction rate will result in a credit to the City of $5,000.
Annual Survey. TSG will annually survey a representative sample of the Airline Group's personnel who use the TSG Services to evaluate their satisfaction relating to the TSG Services. TSG and AMERICAN will agree in writing to the survey's sample group, format, objectives, measures of satisfaction, and desired levels of satisfaction. If such survey reveals that satisfaction has fallen significantly below desired levels, TSG shall develop a plan to improve satisfaction in those areas where satisfaction has fallen below such levels. TSG shall present the plan to AMERICAN within six (6) weeks following compilation of the survey results.
