Amendments to PHI. To the extent that Business Associate maintains PHI in a Designated Record Set, Business Associate shall make any amendment(s) to the PHI in such Designated Record Set that Covered Entity directs or agrees to pursuant to 45 C.F.R. § 164.526, at the request of Covered Entity or an Individual, and in the time and manner reasonably specified by Covered Entity.

Amendments to PHI. Business Associate shall have no obligation to initiate or make PHI amendments to other Participants on the Network. Business Associate shall not respond directly to requests from Individuals for amendments to their PHI. Business Associate will refer such Individuals to the relevant Covered Entity so that the Covered Entity can coordinate and prepare a timely response to the Individual.
Amendments to PHI. If and to the extent that Business Associate maintains PHI in a designated record set, Business Associate shall upon request by Covered Entity make that PHI available to Covered Entity within [insert number] business days for amendment, and shall promptly incorporate any amendments to PHI directed by Covered Entity, as and to the extent required for Covered Entity’s compliance with 45 C.F.R. § 164.52686. If Business Associate receives an individual’s request for an amendment to PHI, Business Associate shall forward that request to Covered Entity within [insert number] business days. Covered Entity shall be responsible for making all determinations regarding the granting or denial of an individual’s request, and for notifying individuals thereof, and Business Associate shall not make any such determinations or make any such amendments except as directed by Covered Entity [Reference: 45 C.F.R. § 164.504(e)(2)(ii)(F)].
Amendments to PHI. The Business Associate shall make an individual’s PHI available for amendment and shall incorporate any amendments to the PHI in accordance with 45 C.F.R. §164.526. Further, within twenty (20) days of the Business Associate’s receipt of the Covered Entity’s request, the Business Associate shall provide the Covered Entity with the PHI that an individual seeks to amend pursuant to 45 C.F.R. §164.526.
Amendments to PHI. Business Associate agrees in the time and manner designated by Covered Entity to make PHI contained in a Designated Record Set available for any amendments that Covered Entity agrees to make pursuant to 45 CFR § 164.526 or to otherwise allow Covered Entity to comply with its obligations under 45 CFR § 164.526. If any Individual requests an amendment of PHI, or a record about the Individual, contained in a designated Record Set directly from Business Associate or its agents or subcontractors, Business Associate shall notify Covered Entity in writing within five (5) days of the request. Covered Entity shall have sole authority and responsibility to approve or deny such a request, and shall notify Business Associate, in writing, of its decision to approve or deny any such request.
Amendments to PHI. Amendment and incorporation of amendments to PHI in accordance with 45 C.F.R. §164.526. Accounting of Disclosures of PHI. Accounting of disclosures made of PHI in accordance with 45 C.F.R. §164.528. Confidential Communication of PHI. Reasonable accommodations to help assure confidential communications in accordance with 45 C.F.R. §164.522(b). Restrictions on PHI. Restrictions put in place to limit certain uses and/or disclosures in accordance with 45 C.F.R. §164.522(a). Privacy Notice. Business Associate shall implement and maintain the following: Post the Kent County Health Department privacy notice or Business Associate privacy notice at each site where PHI is used and/or disclosed to individuals, using a wall or public surface that is easily accessible to clients. Distribute the Kent County Health Department Privacy Notice or Business Associate privacy notice to each individual offering PHI or potentially giving PHI. Make a good faith effort to obtain a signed Proof of Receipt form from each individual that receives a Privacy Notice, and document the outcome of those good faith efforts. File and archive Proof of Receipt forms for a minimum of six (6) years.
Amendments to PHI. If Business Associate receives a Patient request for an amendment to PHI, Business Associate shall, within five (5) business days of its receipt of the request, notify the Covered Entity responsible for making the determination regarding the granting or denial of a Patient’s request. Covered Entity shall be responsible for making all determinations regarding the granting or denial of a Patient’s request for amendment, and for notifying Patients thereof, and Business Associate shall not make any that determinations or make any that amendments. In the event that Business Associate identifies multiple covered entities to which the Patient could make the request, then Business Associate shall coordinate discussion amongst the covered entities to determine the appropriate course of action. If and to the extent that Covered Entity requests Business Associate’s assistance or directs Business Associate to incorporate an amendment to PHI, Business Associate shall, within ten (10) days of the Covered Entity’s request, cooperate with the Covered Entity, including incorporating any amendments to PHI directed by Covered Entity, as and to the extent required for Covered Entity’s compliance with 45 C.F.R. § 164.526.

Amendments to PHI. Subject to Customer’s obligations in Section 4 regarding Qlik Cloud Customer Content, Qlik agrees to make any amendment(s) to PHI in a Designated Record Set that the Covered Entity directs or agrees to pursuant to 45 C.F.R. § 164.526 at the request of Covered Entity or an Individual, and in the time and manner reasonably designated by the Covered Entity.
Amendments to PHI. If Business Associate maintains PHI in a Designated Record Set, promptly (not later than ten (10) business days) following Covered Entity's request, Business Associate shall make any amendments or corrections to PHI that Covered Entity directs or agrees to pursuant to §164.526 of the Privacy Rule. Business Associate shall make available protected health information for amendment and incorporate any amendments to protected health information in accordance with section 164.526.
Amendments to PHI. If applicable, Contractor agrees to make any amendment(s) to PHI in a Designated Record Set as directed or agreed to by Covered Entity or RC pursuant to 45 C.F.R. section 164.526, and as requested by the Covered Entity, RC or a Consumer, within fifteen (15) days of receipt of a request. Any denials, in whole or in part, of requested amendments shall be made by Contractor in accordance with 45 C.F.R. section 164.526.
