ALLEGED VIOLATIONS.
1. Failure to provide complete, timely, and accurate information during the application, interview, matching, and SOAP processes;
2. Discrepancies in graduation credentials;
3. Attempts to subvert or circumvent eligibility requirements, the matching process, or SOAP;
4. Failure to offer or accept an appointment as required by the results of a Match outcome;
5. Failure to engage in ethical and/or professionally responsible behavior; or
6. Any other irregular behavior or activity that occurs in connection with registration, the submission or modification of a rank order or SOAP preference list, and/or the participant's commitment to honor the Match outcome.
ALLEGED VIOLATIONS. At its discretion, the NRMP will investigate alleged violations of this Agreement, including but not limited to:
1. Failure to provide complete, timely, and accurate information during the application, interview, and matching processes;
2. Attempts to subvert or circumvent eligibility requirements or the matching process;
3. Failure to accept an appointment as required by the results of a Fellowship Match outcome;
4. Failure to engage in ethical and/or professionally responsible behavior; or
5. Any other irregular behavior or activity that occurs in connection with registration, the submission or modification of a rank order list, and/or the applicant’s commitment to honor the Match outcome.

ALLEGED VIOLATIONS.

2. Misrepresentation of graduation credentials;
3. Attempts to subvert or circumvent eligibility requirements, the matching process, or SOAP;
4. Failure to offer or accept an appointment as required by the results of a Match outcome;
5. Failure to engage in ethical and/or professionally responsible behavior; or
6. Any other irregular behavior or activity that occurs in connection with registration, the submission or modification of a rank order or SOAP preference list, and/or the participant's commitment to honor the Match outcome. The NRMP’s decision whether to investigate an alleged violation is final and not subject to appeal.
ALLEGED VIOLATIONS. EPA alleges that between July 1, 2015 and September 26, 2016, Respondent violated CWA Section 301(a), 33 U.S.C. § 1311(a) on at least six (6) days by discharging pollutants from a point source into waters of the United States without NPDES permit authorization.
ALLEGED VIOLATIONS. (i.) LTC Facility shall notify the Hospice administrator immediately upon notification of alleged violations involving mistreatment, neglect, or verbal or mental, sexual and physical abuse, including injuries of unknown source and misappropriation of resident property by Hospice personnel.
(ii.) LTC Facility shall conduct investigations of allegations and report to regulatory and other authorities in accordance with federal, state and local laws and per facility policy and procedure.

ALLEGED VIOLATIONS. Alleged violations of this provision shall be pursued through the appropriate state or federal court or administrative agency, and not through the grievance process.
ALLEGED VIOLATIONS. Company reserves the right to terminate your use of the Service and/or the Site. To ensure that Company provides a high quality experience for you and for other users of the Site and the Service, you agree that Company or its representatives may access your account and records on a case-by-case basis to investigate complaints or allegations of abuse, infringement of third party rights, or other unauthorized uses of the Site or the Service. Company does not intend to disclose the existence or occurrence of such an investigation unless required by law, but Company reserves the right to terminate your account or your access to the Site immediately, with or without notice to you, and without liability to you, if Company believes that you have violated any of the Terms of Use, furnished Company with false or misleading information, or interfered with use of the Site or the Service by others.
