ACCREDITATION REQUIREMENTS. Participating pharmacies must have a designated named pharmacist, agreed by the PCT, who will have the clinical responsibility for dispensing and supervision arrangements, ensuring necessary procedures for accuracy and safety are in place. The designated pharmacist must ensure that appropriate arrangements are in place to minimise risk to clients, provider, pharmacy staff and other patients. It is essential to have a Standard Operating Procedure (SOP) in place. Supervision should take place in a designated area offering suitable privacy for the client, other customers and the general public. Supervision must never take place in the dispensary. ACCREDITATION REQUIREMENTS. The system at Majuba Power Station is a TOA SX-2000 series Voice Evacuation system. The system is under extended warranty from the OEM. Contractors are required to be OEM accredited to work on the TOA SX-2000 series of Voice Evacuation equipment. Only OEM accredited technical personnel shall be permitted to perform work on the system. Submission of the contractors OEM accreditations as part of the Tender Returnable for the purposes of technical evaluation is mandatory.

The Owner must: maintain its Accreditation and comply with all conditions of its Accreditation prior to the Commitment Date and thereafter until the Expiry Date; or if the Owner is not the Rail Infrastructure Manager for the Private Infrastructure, ensure that the Rail Infrastructure Manager for the Private Infrastructure maintains its Accreditation and complies with all conditions of its Accreditation prior to the Commitment Date and thereafter until the Expiry Date, notify Aurizon Network immediately if at any time during the term of this Agreement the Rail Infrastructure Manager for the Private Infrastructure ceases to be Accredited in respect of the Private Infrastructure. Aurizon Network must: be Accredited as the Rail Infrastructure Manager for the Connecting Infrastructure and the Network prior to the Commitment Date and thereafter until the Expiry Date; and notify the Owner immediately if at any time during the term of this Agreement it has ceased to be Accredited in respect of the Connecting Infrastructure or the Network.
