Accident Investigation. All accidents resulting in a fatality or injury requiring medical treatment will be investigated by one representative of the IBEW and one representative of the Employer. The accident investigation report will be submitted to:
Accident Investigation. In the event I witness or am involved in an accident or event resulting in property damage or personal injury of any kind, no matter how insignificant it may seem, I agree to immediately report such accident or event to a Liberty University employee or a Camp staff member and to cooperate in the investigation of the accident or event.
Accident Investigation. In the event of a serious accident the Human Resources Department will notify the designated Union member of the Joint Safety Committee in order to make a joint accident investigation. The parties agree to implement a joint accident investigation team as soon as practical. The team will be responsible for joint investigation of all lost time accidents. Accident Investigation. In the event of any accident, loss or casualty event involving any use of the Equipment, whether resulting in property damage or bodily injury or other loss or damage, Customer shall, under Supplier and/or Supplier's insurer's direction, provide, at its sole cost and expense, for the full and complete investigation of any such accident, loss or casualty event. Costs and expenses of any investigation may include, but shall not be limited to, reasonable fees and costs for, lawyers, experts, forensic investigations, adjusters, and inspection, disassembly, transportation and/or storage of the Equipment. The Customer agrees that it will pay all defense costs of investigations conducted by Supplier, or entities working on Supplier’s behalf.
Accident Investigation. All accident/incidents shall be investigated by the contractor’s safety supervisor and/or their safety designee. An accident investigation report must be submitted to the Designated Project Representative, OCIP Administrator/Xxxxxx Xxxxxx Xxxxxx and OCIP Insurance Carrier within twenty-four (24) hours of the occurrence. The accident investigation should generate appropriate recommendations for corrective actions to prevent recurrence of similar accidents. Depending upon severity of the accident, the xxxxxxx of the injured worker may be requested to appear at the job safety and coordination meeting to:
Accident Investigation. Every injury or near miss which involves or would have involved a worker going to a doctor or hospital must be investigated. The union co-chair or the health and safety rep shall be made available in a timely manner to investigate the accident or incident along with the supervisor or manager of the area.
Accident Investigation. Accident Investigation reports are required for all employee injuries (no matter how minor), occupational-related illnesses, property damage, general liability, near miss incidents, pollution incidents, and environmental hazards or incidents. Accident Investigation Reports must be completed within 24 hours of the incident unless the ESCO or Subcontractor notifies the Superintendent of the ESCO or the xxxxxxx or superintendent of the Subcontractor that the circumstances surrounding the incident will require additional investigation. Failure to complete an Accident Investigation Report within 24 hours of an occurrence shall result in a $250 penalty. This penalty shall be issued through a Non-Negotiable Deduct Change Order or other means approved by the School District.
