Accident Care. Any student injured during school hours or while participating in school- sponsored activities or athletics, must report the injury to an instructor and/ or the office immediately. An accident report is filled out and parents are notified when necessary. If the injury requires professional medical attention, parents will be asked to pick up their child at school or at a medical facility. If a student must be transported by ambulance to a hospital, a school official will accompany him or her until a parent appears at the hospital. The hospital must have prior parent/guardian approval before treating a minor. Concussion Awareness A concussion is a type of traumatic brain injury that changes the way the brain normally works. A concussion is caused by a fall, bump, blow, or jolt to the head or body that causes the head and brain to move quickly back and forth. A fact sheet about the symptoms and what to do if a concussion is suspected appears on the district Web site xxx.xxxxxx00.xxx under the forms section of the athletics tab. Contact your building principal if further information is needed. Student Insurance Utica Community Schools provides a free basic accident insurance policy for students that covers them at school and while participating in school- sponsored events. A student must report any accident to the school office immediately. To collect benefits, a parent or guardian must complete a claim form. Only expenses not covered by family, employer or group insurance will be considered. Accident victims may be required to pay a deductible. The benefit period covers 52 weeks from the date of injury.   • Dependent Care The College will make available to employees, at their option, an Internal Revenue Service Code Section 129 Dependent Care plan. The plan will be established, administered, and communicated to employees by the State without cost to the employees.

  • Transportation of Accident Victims Transportation to the nearest physician or hospital for employees requiring medical care as a result of an on-the-job accident shall be at the expense of the Employer.

  • Industrial Accident or Illness Leave 7.6.1 All unit members shall receive sixty (60) days’ leave with pay in any one fiscal year for an industrial accident or illness. An industrial accident or illness is defined as one where the unit member becomes ill or is injured while he/she is serving the District, and, the accident or illness is reported to the District’s Self-Insurance Program in accordance with District regulations, and, the District Self-Insurance Program accepts responsibility for the treatment of the unit member.

  • Accident INVESTIGATIONS Whenever an accident occurs involving the equipment or personnel of a Supporting Party, the Protecting Party shall take immediate steps to notify the Supporting Party that an accident has occurred. As soon as practical, the Protecting Party shall initiate an investigation of the accident. A team made up of appropriate representatives from all affected agencies shall conduct the investigation. Costs for investigation personnel are Party-specific and will be borne by the sending Party. Other accident or incident investigation costs are the fiscal responsibility of the Party (ies) that has jurisdiction and/or investigative responsibility. The sharing of information between Parties on accident investigations and their findings and probable causes is a valuable tool for safety and must be encouraged.
