Access Log. The Administration shall maintain a log of all non-University employees who have accessed official personnel files and shall send a copy of the log to the Association each month provided a new entry has been made in the log during that month. In addition, the Administration shall immediately notify individual faculty members when someone not employed by the University has examined their official personnel files.

Access Log. CONTRACTOR shall develop and maintain a written Access Log of all 22 requests for services received via telephone, in writing, or in person. CONTRACTOR is responsible for 23 this written log that meets the DHCS regulations and requirements, as interpreted by the County, and 24 records all services requested twenty-four (24) hours-seven (7) days a week. The Access Log shall 25 contain, at a minimum, whether or not the caller has Medi-Cal, the name of the individual, date of the 26 request, nature of the request, call status (emergent, urgent, routine), if the request is an initial request 27 for Specialty Mental Health Services, and the disposition of the request, which shall include 28 interventions. CONTRACTOR must be able to produce a sortable log, for any time-period specified by 29 County within twenty-four (24) hours of receiving the request from County. If the caller’s name is not 30 provided, then the log shall reflect that the caller did not provide a name. CONTRACTOR shall make 31 available to ADMINISTRATOR upon request, the most recent telephone log which shall include 32 previous day’s calls.
