Access by Individuals. Business Associate shall allow individuals who are the subject of the PHI to inspect and copy their PHI in the possession of Business Associate if Covered Entity does not also maintain such information.
Access by Individuals. Information will not be kept in a designated record set as defined by HIPAA. Requests, if any, from Individuals for access or amendment to their PHI in a designated record set as defined under HIPAA will be referred to the relevant Participant so that the Participant can prepare a timely response to the Individual.
Access by Individuals. 1. Access to the texts by an individual person is to be controlled by that person's organization. The organization may only grant access to people working under its control, i.e., its own members, consultants to the organization, or individuals providing service to the organization.

In accordance with 45 C.F.R. § 164.524 and within thirty (30) days of the request, Business Associate will make available to Covered Entity (or to the individual at Covered Entity’s direction) an individual’s PHI as maintained in a designated record set by Business Associate in the format requested, including provide an electronic copy of the PHI as requested, to the extent possible.
Access by Individuals. Within ten (10) calendar days of receipt of a request by City, Contractor shall permit any individual whose PHI is maintained by Contractor to have access to and to copy his or her PHI, in the format requested, unless it is not readily producible in such format, in which case it shall be produced in hard copy format. In the event any individual requests access to PHI held by Contractor directly from Contractor, Contractor shall, within two (2) days forward such request to City. Any denial of access to the PHI requested shall be the responsibility of City.
Access by Individuals. Business Associate shall at the request of Company, and in the time and manner designated by Company, make available PHI in a designated record set to Company or as directed to an Individual or his/her personal representative entitled to access and copy the PHI in order to meet the requirement of applicable law and regulations.

Requests if any from Individuals for access or amendment to their PHI in a Designated Record Set as defined under HIPAA shall be referred to the relevant Participant so that the Participant can prepare a timely response to the Individual.

Business Associate agrees to promptly make available within five (5) business days or less Protected Health Information in a Designated Record Set to the Covered Entity or, if so directed by the Covered Entity, to the Individual or Individual’s designee as necessary to satisfy Covered Entity’s obligations under 45 C.F.R. §164.524.
